摘 要:为提高布鲁氏菌病检测诊断能力,采用5种布鲁氏菌病抗体竞争酶联免疫吸附试验(cELISA)试剂盒对背景清楚的166份牛、羊、猪血清样本进行检测,同时以试管凝集试验(SAT)作为参照方法,测定5种试剂盒的敏感性、特异性、Youden指数及AUC值等参数。结果显示,5种试剂盒的敏感性为88.00%~100.00%;特异性为86.21%~99.14%;Youden指数为86.21%~95.69%;AUC值为0.931~0.978;McNemar检验结果显示,5种试剂盒中厂家A、C和E检验结果与SAT结果差异不显著(P>0.05),厂家B和D检验结果与SAT结果差异显著(P<0.05);Kappa一致性检验分析表明,5种试剂盒与SAT结果均有高度的一致性(0.790~0.930);成本-效果分析表明,5种试剂盒优先选择依次为A、C、B、E、D。本试验结果为诊断用试剂盒选择提供参考,从而提高诊断准确性。To improve the ability of detection and diagnosis of brucellosis,five cELISA kits were used to detect 166 serum samples for brucellosis antibodies from cattle,sheep and pigs with a clear background.Meanwhile,the sensitivity,specificity,Youden’s J values and AUC values of the five kits were determined by SAT as a reference method.The results showed that the sensitivity of the five kits was 88.00%~100.00%;the specificity was 86.21%~99.14%;the Youden’s J value was 86.21%~95.69%;the AUC value was 0.931~0.978.McNemar test results showed no significant statistical difference was observed between the results of manufacturers A,C,and E with SAT results(P>0.05),while significant difference was observed between the SAT results of manufacturer B and D(P<0.05).Kappa consistency test analysis showed that the five kits were highly consistent with SAT results(0.790~0.930).The cost-effectiveness analysis showed that the priority of the five kits was A,C,B,E and D.These results will provide a reference for the selection of diagnostic kits,thereby improving the diagnostic accuracy.
