摘　　要：目的探讨血管外科血透通路日间手术临床路径应用效果。方法回顾性收集某院血管外科2017年1月-2019年12月采取临床路径血透通路日间手术治疗的患者为临床路径组,与2014年1月-2016年12月尚未采用日间手术临床路径治疗的患者(传统治疗组)进行比较。结果两组患者基线水平无明显差异。对于自体动静脉瘘手术(AVF)患者,临床路径组住院时间和住院总费用明显低于传统治疗组(P<0.05);对于自体动静脉瘘或人工血管动静脉瘘腔内血管成形术(PTA)患者,临床路径组住院时间明显低于传统治疗组(P<0.05)。结论血透通路日间手术临床路径模式可以缩短住院时间,对于AVF患者还可以进一步降低住院费用,具有实际推广价值。Objective To explore the application effect of the clinical pathway of ambulatory surgery of hemodialysis access.Methods The data of the patients who received the clinical pathway of ambulatory surgery of hemodialysis access in a hospital from January 2017 to December 2019 were collected and compared with the data of the patients who did not use the clinical pathway(traditional treatment group)from January 2014 to December 2016.Results There was no significant difference in baseline level between the two groups.For patients with autologous arteriovenous fistula(AVF),the length of stay and total cost of hospitalization in the clinical pathway group were significantly lower than those in the traditional treatment group(P<0.05);for patients with percutaneous transluminal angioplasty(PTA),the length of stay in the clinical pathway group was significantly lower than that in the traditional treatment group(P<0.05).Conclusion The ambulatory surgery mode of hemodialysis access under clinical pathway can shorten the hospitalization day,and further reduce the hospitalization cost for AVF patients,which has the value of implementation and promotion.
