摘  要:目的探究胱抑素C(CysC)、β2-微球蛋白(β2-MG)、白细胞介素-1β(IL-1β)、白细胞介素-18(IL-18)对高血压患者早期肾损伤的预测价值。方法选取2015年3月至2020年9月本院高血压早期肾损伤患者74例为观察组,及同期单纯高血压患者36例为对照组。比较两组CysC、β2-MG、IL-1β、IL-18,分析CysC、β2-MG、IL-1β、IL-18与血压相关性、肾损伤影响因素及各指标对肾损伤的预测价值。结果两组年龄、性别、体质量指数、TC、TG、FBG、LDL-C、HDL-C、尿酸、尿素氮比较,差异无统计学意义(P>0.05);两组高血压病程、血压控制情况、SBP、DBP比较,差异有统计学意义(P<0.05);观察组CysC、β2-MG、IL-1β、IL-18水平高于对照组,差异有统计学意义(P<0.05);CysC、β2-MG、IL-1β、IL-18与SBP、DBP呈正相关(P<0.05);CysC、β2-MG、IL-1β、IL-18是高血压早期肾损伤的重要危险因素(P<0.05);CysC、β2-MG、IL-1β、IL-18单一检测AUC值分别为0.798、0.842、0.753、0.788,联合预测AUC为0.854,略高于β2-MG单独预测,显著高于CysC、IL-1β、IL-18单独预测,差异有统计学意义(P<0.05)。结论CysC、β2-MG、IL-1β、IL-18与高血压患者血压水平高度相关,联合检测水平变化可作为临床预测早期肾损伤发生的重要途径,对诊治预防具有重要应用价值。Objective To explore the predictive value of cystatin C(CysC),β2-microglobulin(β2-MG),interleukin-1β(IL-1β),and interleukin-18(IL-18)in early renal injury in patients with hypertension.Methods A total of 74 patients with hypertension and early renal injury in our hospital from March2015 to September 2020 were selected as the observation group,and 36 patients with simple hypertension during the same period were selected as the control group.The CysC,β2-MG,IL-1β,IL-18 in the two groups were compared.The correlation between CysC,β2-MG,IL-1β,IL-18 and blood pressure,the influencing factors of renal injury and the predictive value of each indicator on renal injury were analyzed.Results There was no significant difference in age,gender,body mass index,TC,TG,FBG,LDL-C,HDL-C,uric acid,and urea nitrogen between 2 groups(P>0.05).The duration of hypertension and blood pressure control in 2 groups were compared with SBP,DBP,the difference was statistically significant(P<0.05).The levels of CysC,β2-MG,IL-1βand IL-18 in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Cys C,β2-MG,IL-1β,IL-18 are positively correlated with SBP and DBP(P<0.05).Cys C,β2-MG,IL-1β,IL-18 are important risk factors for early renal injury in hypertension(P<0.05).Cys C,β2-MG,IL-1β,IL-18 single detection of AUC values is 0.798,0.842,0.753,0.788.The combined predicted AUC is 0.854,which is slightly higher than that ofβ2-MG alone and significantly higher than that of Cys C,IL-1β,and IL-18 alone,and the difference was statistically significant(P<0.05).Conclusion Cys C,β2-MG,IL-1β,and IL-18 are highly correlated with blood pressure levels in patients with hypertension.Combined detection of changes in levels can be used as an important way to predict the occurrence of early renal injury in clinic,and has important application value for diagnosis,treatment,and prevention.
