股骨近端螺旋刀片抗旋髓内钉内固定在股骨粗隆间骨折的临床研究

摘　　要：目的探讨股骨近端螺旋刀片抗旋髓内钉内固定(PFNA)治疗股骨粗隆间骨折的临床效果。方法将我院骨科2018年1月至2020年1月收治的60例股骨粗隆间骨折患者纳入本研究,采用随机数字表法分成两组,每组各30例,其中对照组应用动力髋螺钉(DHS)内固定,观察组应用PFNA,比较两组手术时间、术中出血量、住院时间、骨折愈合时间、Harris髋关节功能评分、髋关节功能优良率及并发症发生率。结果观察组术中出血量低于对照组,手术时间长于对照组,住院时间、骨折愈合时间短于对照组,Harris评分高于对照组,差异均有统计学意义(P<0.05);观察组髋关节功能优良率为96.67%,高于对照组的80.00%,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论股骨粗隆间骨折治疗期间应用PFNA内固定临床效果优良,虽然手术耗时更长,但出血量更少,住院时间与骨折愈合时间更短,还可有效改善髋关节功能,故而值得推广。Objective To investigate the clinical efficacy of internal fixation of proximal femoral nail antirotation(PFNA)in the treatment of intertrochanteric fractures.Methods A total of 60 patients with intertrochanteric fractures admitted to the department of orthopedics in our hospital from January 2018 to January 2020 were included in this research,and they were divided into the control group(n=30)and the observation group(n=30).The control group was treated with internal fixation of dynamic hip screw(DHS),while the observation group was treated with PFNA.The operation time,intraoperative hemorrhage volume,hospitalization time,fracture healing time,Harris hip joint function score,good rate of hip joint function and the incidence of complications were compared between the two groups.Results The intraoperative hemorrhage volume in the observation group was lower than that in the control group.The operation time was longer than that in the control group.The hospitalization time and fracture healing time were shorter than those in the control group.And the Harris score was higher than that in the control group.All the above indexes were with statistically significant differences(P<0.05).The good rate of hip joint function in the observation group was 96.67%,higher than 80.00%in the control group,with statistically significant difference(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The clinical efficacy of PFNA internal fixation during the treatment of intertrochanteric fractures is excellent.Although the operation takes more time,it has less hemorrhage volume,shorter hospitalization time and fracture healing time,and can effectively improve the hip join t function,so it is worthy of promotion.
