芪参益心汤联合常规西药治疗气虚血瘀型冠心病心绞痛63例

摘　　要：目的:观察芪参益心汤联合常规西药治疗气虚血瘀型冠状动脉粥样硬化性心脏病(简称冠心病)心绞痛的临床疗效。方法:将126例冠心病心绞痛气虚血瘀证患者按照随机数字表法分为对照组和治疗组,每组各63例。对照组给予常规西医治疗,治疗组在对照组的基础上加以芪参益心汤治疗。比较两组患者的临床疗效,检测两组患者治疗前后总胆固醇(total cholesterol,TC)、三酰甘油(triacylglycerol,TG)、高密度脂蛋白(high density lipoprotein cholesterol,HDL-C)、低密度脂蛋白(low density lipoprotein cholesterol,LDL-C)、载脂蛋白(apolipoprotein,Apo)B、ApoA1、ApoB/A1、一氧化氮(nitric oxide,NO)及内皮素(endothelin,ET)水平。测量两组患者治疗前后左心室射血分数(left ventricular ejection fraction,LVEF)、心输出量(cardiac output,CO)、胸腔液体含量(thoracic fluid content,TFC)以及体循环血管阻力(systemic vessel resistence,SVR)等心功能指标。结果:对照组有效率为73.02%,治疗组有效率为87.30%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后LVEF、CO、NO、TC、TG、LDL-C、ApoB及ApoB/A1水平较高于本组治疗前,HDL-C、ApoA1、TFC、SVR、ET水平低于本组治疗前,治疗组治疗后LVEF、CO、NO、TC、TG、LDL-C、ApoB及ApoB/A1水平较高于对照组治疗后,HDL-C、ApoA1、TFC、SVR、ET水平低于对照组治疗后,差异均有统计学意义(P<0.05)。结论:芪参益心汤联合常规西医治疗气虚血瘀型冠心病心绞痛,能明显改善患者的脂代谢水平和心脏功能。Objective:To observe the clinical curative effect of Astragalus and Ginseng Heart-Boosting Decoction combined with conventional Western medicine on angina pectoris of coronary heart disease with qi deficiency and blood stasis syndrome.Methods:A total of 126 patients with angina pectoris with qi deficiency and blood stasis syndrome were randomly divided into the control group and the treatment group,with 63 cases in each group.The control group was treated with conventional Western medicine,while the treatment group was treated with Astragalus and Ginseng Heart-Boosting Decoction on the basis of the control group.The clinical effects of the two groups were compared.Total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),apolipoprotein(Apo)B,ApoA1,ApoB/A1,nitric oxide(NO)and endothelin(ET)levels were detected before and after treatment.Left ventricular ejection fraction(LVEF),cardiac output(CO),thoracic fluid content(TFC),systemic vascular resistance(SVR)and cardiac function indexes were measured before and after treatment.Results:The effective rate of the control group was 73.02%,while that of the treatment group was 87.30%,and the difference between the two groups was statistically significant(P<0.05).After treatment,the levels of LVEF,CO,ACI,NO,TC,TG,LDL-C,ApoB and apoB/A1 in the two groups were much higher than those of the same group before treatment.The levels of HDL-C,ApoA1,TFC,SVR and ET were lower than those of the same group before treatment.The levels of LVEF,CO,ACI,NO,TC,TG,LDL-C,ApoB and apoB/A1 in the treatment group were higher than those in the control group after treatment,while the levels of HDL-C,ApoA1,TFC,SVR and ET were lower than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of angina pectoris of coronary heart disease with qi deficiency and blood stasis syndrome,Astragalus and Ginseng Heart-Boosting Decoction combined with conventional Western medicine can sign
