摘　　要：目的探讨经皮脊柱内镜日间手术治疗青壮年腰椎间盘突出症的安全性及其社会意义。方法遵循严格的纳入、排除标准,将2018年6月至2019年6月郑州市骨科医院收治的168例青壮年腰椎间盘突出症患者纳入研究,78例采用经皮脊柱内镜日间手术治疗的腰椎间盘突出症患者作为观察组,以同期90例常规住院管理的采用经皮脊柱内镜治疗的腰椎间盘突出症的患者作为对照组。对两组患者的临床疗效、并发症、住院时间、人均住院费用、再入院率等方面进行比较。结果两组手术均顺利完成。两组临床疗效、术后并发症及再入院率方面比较差异未见统计学意义(P>0.05)。与对照组比较,观察组住院时间、人均住院费用显著减少,差异有统计学意义(P<0.05)。结论经皮脊柱内镜日间手术治疗青壮年腰椎间盘突出症是安全、可行的,能显著缩短患者住院时间、降低住院费用,达到方便患者、节约费用、提高医疗资源使用效率的目的。Objective To investigate the safety and social significance of the percutaneous endoscopic lumbar discectomy ambulatory surgery in the treatment of lumbar disc herniation in young adults.Methods According to strict inclusion and exclusion criteria,168 cases of lumbar disc herniation in young adults treated in our department from June 2018 to June 2019 were included in the study and divided into two groups,the control group(n=90)and the observation group(n=78).The control group received percutaneous endoscopic lumbar discectomy in inpatient setting,while the observation group received percutaneous endoscopic lumbar discectomy ambulatory surgery.Then the clinical efficacy,complications,length of stay,per capita cost of hospitalization,readmission rate of the two groups were compared.Results The operation was successfully completed in both groups.There was no significant difference in clinical efficacy,postoperative complications or readmission rate between the two groups(P>0.05).Compared with the control group,the length of stay and per capita cost of hospitalization in the observation group were significantly reduced(P<0.05).Conclusions It is safe and feasible to treat lumbar disc herniation in young adults by ambulatory surgery of percutaneous endoscopic lumbar discectomy,and it can shorten the hospitalization time and reduce the hospitalization cost,so as to achieve the purpose of convenient patients,saving costs and improving the efficiency of medical resources use.
