心电图碎裂QRS波对冠心病的诊断价值

摘　　要：目的探讨心电图碎裂QRS波对冠心病的诊断价值。方法选择2019年1月至2019年12月济源市人民医院收治的300例疑似冠心病(CAD)患者为研究对象,均行冠状动脉造影术及心电图检查,将冠状动脉造影术的结果作为金标准,比较两组患者心电图破裂QRS波的诊断效能,比较患者间的血管支数、罪犯血管、狭窄程度心电图破裂QRS检测率。结果CAD组患者中冠状动脉多支病变心电图碎裂QRS波检出率显著高于单支病变及双支病变的心电图碎裂QRS波检出率(P<0.05)。不同冠状动脉病变血管与心电图碎裂QRS波检出率不具有相关性(P>0.05)。冠状动脉完全闭塞心电图碎裂QRS波检出率为92.59%,显著高于中度狭窄及重度狭窄的心电图碎裂QRS波检出率(P<0.05)。结论针对CAD患者的诊断,应用心电图碎裂QRS能够有效预测其多支病变,可作为临床辅助诊断CAD的一项重要指标。Objective To investigate the value of QRS complex in the diagnosis of coronary heart disease with fragmented ECG.Methods Three hundred cases of suspected CAD patients admitted to Jiyuan People’s Hospital from January 2019 to December 2019 were selected as the objects.All patients underwent coronary angiography and ECG examination.The results of coronary angiography were taken as the gold standard.The diagnostic efficiency of ECG rupture QRS complex was compared between the two groups,and the number of vessels,culprit vessels,stenosis degree and detection rate of ECG rupture QRS between the two groups were compared.Results The QRS detection rate of electrocardiogram fragmentation of multiple coronary artery lesions in the CAD group was significantly higher than that of single-vessel lesions and double-vessel lesions(P<0.05).There was no significant correlation of the detection rate of fragmented QRS wave with different coronary artery lesions(P>0.05).The detection rate of fragmented QRS complex in complete occlusion of coronary artery was 92.59%,which was significantly higher than that in moderate stenosis and severe stenosis(P<0.05).Conclusions For the diagnosis of CAD patients,the application of ECG fragmented QRS can effectively predict the multi vessel lesions,which can be used as an important index for clinical auxiliary diagnosis of CAD.
