艾灸联合中药对多囊卵巢综合征不孕症患者卵巢储备功能的影响

摘　　要：目的观察艾灸联合紫仙助孕汤对多囊卵巢综合征不孕症患者卵巢储备功能的影响。方法80例多囊卵巢综合征不孕症患者,按照随机抽签法分为对照组和联合组,每组40例。对照组给予枸橼酸氯米芬治疗,联合组在对照组基础上给予艾灸联合紫仙助孕汤加减治疗。观察两组治疗前后卵巢储备功能相关指标[窦卵泡计数、血清抗苗勒管激素(AMH)和抑制素B(INHB)]、性激素水平[睾酮、雌二醇、促黄体激素(LH)、促卵泡激素(FSH)]、卵巢体积、子宫内膜厚度变化,并比较两组临床疗效及排卵率和妊娠率。结果两组治疗后窦卵泡计数、卵巢体积、血清睾酮和LH水平低于治疗前,血清AMH、INHB、雌二醇、FSH水平和子宫内膜厚度高于治疗前;联合组治疗后窦卵泡计数、卵巢体积、血清睾酮和LH水平低于对照组,血清AMH、INHB、雌二醇、FSH水平和子宫内膜厚度高于对照组,差异有统计学意义(P<0.05)。联合组总有效率、排卵率、妊娠率高于对照组,差异有统计学意义(P<0.05)。结论在西药治疗基础上,艾灸联合紫仙助孕汤能有效改善多囊卵巢综合征不孕症患者卵巢储备功能和性激素水平,促进成熟卵泡发育和子宫内膜生长,有助于提高患者排卵率和妊娠率。Objective To observe the effect of moxibustion plus Zi Xian Zhu Yun decoction on ovarian reserve function in patients with infertility due to polycystic ovary syndrome(PCOS).Method Eighty PCOS-associated infertile patients were divided into a control group and an integration group by the random sampling method,with 40 cases in each group.The control group was treated with Clomifene citrate,while the integration group was given additional moxibustion treatment plus modified Zi Xian Zhu Yun decoction.The indexes related to ovarian reserve function[antral follicle count(AFC),serum anti-Mullerian hormone(AMH)and inhibin B(INHB)],sex hormone levels[testosterone(T),estradiol(E2),luteinizing hormone(LH)and follicle-stimulating hormone(FSH)],ovary volume and endometrial thickness were observed before and after treatment in the two groups.The clinical efficacy,ovulation rate and pregnancy rate were also compared between the two groups.Result The AFC,ovary volume,and serum T and LH levels decreased after treatment in both groups,while the serum AMH,INHB,E2 and FSH levels and endometrial thickness increased after treatment;the AFC,ovary volume,and serum T and LH levels were lower in the integration group than in the control group after treatment,while the serum AMH,INHB,E2 and FSH levels and endometrial thickness were higher in the integration group,and the differences were statistically significant(P<0.05).The total effective rate,ovulation rate and pregnancy rate were higher in the integration group than in the control group,and the between-group differences were statistically significant(P<0.05).Conclusion Moxibustion plus Zi Xian Zhu Yun decoction on the basis of conventional Western medication can effectively improve the ovarian reserve function and sex hormone levels,promote the development of mature follicles and endometrium,and is beneficial to the ovulation and pregnancy rates in the patients with PCOS-associated infertility.
