摘  要:目的:分析二维超声联合四维超声对胎儿畸形的诊断价值。方法:选择2019年2月—2020年2月98例疑似胎儿畸形的孕产妇,均经二维超声联合四维超声检查,以胎儿出生后或终止妊娠的临床诊断结果为金标准,比较二维和四维超声图像联合诊断及单个诊断的价值。结果:以胎儿出生后或终止妊娠的临床诊断结果为金标准,98例疑似胎儿畸形孕产妇中,18例(18.37%)确诊为胎儿畸形;联合诊断胎儿畸形的灵敏度与二维、四维超声单独检测比较,差异无统计学意义(P>0.05);联合诊断胎儿畸形的特异度、准确度高于二维超声、四维超声单独检测,差异有统计学意义(P<0.05);二维超声检查结果与临床诊断结果具有较理想的一致性(Kappa=0.425);四维超声检查结果与临床诊断结果具有较理想的一致性(Kappa=0.572);联合诊断检查结果与临床诊断结果具有极好的一致性(Kappa=0.785)。结论:二维及四维超声联合应用于胎儿畸形诊断中具有较高的特异度与准确度,可保障新生儿出生质量,具有较高的临床应用价值。Objective:To analyze the value of two-dimensional ultrasound combined with four-dimensional ultrasound in the diagnosis of fetal malformation.Methods:From February 2019 to February 2020,98 pregnant women with suspected fetal malformation were selected and examined by two-dimensional ultrasound combined with four-dimensional ultrasound.The value of two-dimensional and four-dimensional ultrasound combined diagnosis and single diagnosis were compared based on the clinical diagnosis results of the fetus after birth or termination of pregnancy as the gold standard.Results:The gold standard was the clinical diagnosis of the fetus after birth or termination of pregnancy.Among 98 pregnant women with suspected fetal malformation,18 cases(18.37%)were diagnosed as fetal malformation;there was no difference between the sensitivity of combined diagnosis of fetal malformation and that of two-dimensional and four-dimensional ultrasound(P>0.05).The specificity and accuracy of combined diagnosis of fetal malformation were higher than that of two-dimensional and four-dimensional ultrasound(P<0.05).The results of two-dimensional ultrasound were statistically significant(P<0.05).The results of four-dimensional ultrasound were in good agreement with the results of clinical diagnosis(Kappa=0.425).The results of four-dimensional ultrasound were in good agreement with the results of clinical diagnosis(Kappa=0.572).The results of combined diagnosis were in good agreement with the results of clinical diagnosis(Kappa=0.785).Conclusion:The combination of two-dimensional and four-dimensional ultrasound in the diagnosis of fetal malformation has high specificity and accuracy,can guarantee the quality of newborn birth,and has high clinical application value.
