基于高频次多元互动工具的个案动态管理在房颤华法林用药者中的应用

摘　　要：目的:观察基于高频次多元互动工具的个案动态管理在房颤华法林用药者中的应用效果。方法:选择2019年1—2019年6月收治的持续房颤患者80例,按随机数字表法分为观察组和对照组,每组40例。对照组接受常规延续护理,观察组在对照组基础上加用基于高频次多元互动工具的个案动态管理干预。对两组干预后的各指标进行比较。结果:观察组干预后用药认知度评分、国际标准化比值(INR)达标率显著高于对照组,华法林用药不良事件发生率显著低于对照组,差异均有统计学意义(P<0.05)。结论:采用基于高频次多元互动工具的个案动态管理对房颤华法林用药者进行干预,可显著提高该类患者的疾病认知度与INR达标率,降低华法林用药不良事件发生率。Objective:To observe the effect of case dynamic management based on high-frequency multiple interactive tools in patients with atrial fibrillation warfarin.Methods:All 80 patients with persistent atrial fibrillation who were admitted to hospital from January to June 2019 were selected as observation samples.They were divided into 40 experimental groups and 40 control groups by random number table method.The control group received conventional continuing care.On the basis of the group,a case-based dynamic management intervention based on high-frequency multiple interactive tools was used to compare the observation indicators after the two groups of intervention.Results:Patients with persistent atrial fibrillation warfarin in the test group had significantly higher disease cognitive scores and international normalized ratio(INR)compliance rates after intervention than those in the control group,and the incidence of adverse events with warfarin medication was significantly lower than the control group(P<0.05).Results:Patients with persistent atrial fibrillation warfarin in the experimental group had significantly higher disease awareness scores and INR compliance rates after intervention than those in the control group,and the incidence of adverse events with warfarin medication was significantly lower than that in the control group(P<0.05).Conclusion:Intervention of patients with atrial fibrillation warfarin medication using dynamic case-based management based on high-frequency multiple interaction tools can significantly improve the disease awareness and international normalized ratio compliance rate of these patients,and reduce the incidence of adverse events with warfarin medication.
