间质病变为主肺结核的CT表现及诊断

摘　　要：目的探讨以肺间质病变为主肺结核CT表现及特点,提高CT诊断水平。方法收集泰安市肿瘤防治院2018年1月至2019年12月经细菌学、分子生物学或病理组织学证实以间质病变为主要CT表现肺结核患者37例,分析病变形态分布特点和基本CT征象检出率,并进行统计学分析。结果37例患者间质病变位于肺结核好发部位1、2区分别为21、16例,合计37例;非好发部位3、4、5区分别为7、5、4例,合计16例。单纯跨段片状融合分布10例,单纯沿支气管树簇状分布12例,既跨段片状融合分布又沿支气管树簇状分布15例。小叶内细网织影、小叶中心结节、磨玻璃密度影、支气管壁增厚、树芽征、小支气管扩张、腺泡结节、空洞、小叶间隔增厚的检出率分别为100%、100%、89%、73%、70%、51%、40%、32%、30%。结论以肺间质病变为主肺结核CT表现有一定的特征性。Objective To explore the CT manifestations and characteristics of interstitial pulmonary tuberculosis,improve the CT diagnosis.Methods Collecting 37 cases of pulmonary tuberculosis confirmed by bacteriology,molecular biology or histopathology,from January 2018 to December 2019 in Taian cancer prevention and treatment hospital.Analyze the distribution of pathological changes and the detection rate of basic CT signs,and make statistical analysis.Results Among the 37 patients,21 and 16 cases were located in the first and second regions of the tuberculosis predilection sites,37 cases in total;There were 7,5 and 4 cases in the 3,4 and 5 areas of the non predisposing sites,16 cases in total.There were 10 cases in the form of a large fusion lesion,12 cases distributed along the bronchus,15 cases were both of them.Intra-lobular reticular reticulation(100%)、centrilobular nodule(100%)、ground-glass opacity(89%)、thickening of the airway wall(73%)、tree-in-bud(70%)、bronchiectasis(51%)、acinar nodule(40%)、cavity(32%)、thickening of inter-lobular septa(30%).Conclusion CT manifestations of pulmonary tuberculosis with pulmonary interstitial disease as predominate have certain characteristics.
