甲泼尼龙耳后注射联合巴曲酶治疗突发性耳聋的效果

摘　　要：目的探讨甲泼尼龙耳后注射联合巴曲酶治疗突发性耳聋的效果。方法抽取2017年1月至2019年1月河南科技大学第一附院医院收治的98例突发性耳聋患者,采用随机数字分组法分为观察组(50例)与对照组(48例),并且将两组患者按照不同听力受损程度分为A、B、C三亚组,A组为低中频下降,B组为中高频下降,C组为平坦或全聋。观察组A组15例、B组20例、C组15例;对照组A组15例、B组20例、C组13例。对照组给予巴曲酶和常规治疗,观察组在对照组基础上联合运用甲泼尼龙琥珀酸钠耳后局部注射治疗。治疗2周后比较两组及各亚组患者的临床疗效;比较两组患者耳鸣症状的治疗效果和并发症的发生情况。结果治疗后,观察组总有效率(82.00%)高于对照组(56.28%),差异有统计学意义(P<0.05)。观察组A组总有效率为93.33%,B组总有效率(90.00%)高于对照组A组(60.00%)和B组总有效率(60.00%),差异有统计学意义(P<0.05)。观察组耳鸣治疗的总有效率(86.00%)高于对照组(60.25%),差异有统计学意义(P<0.05)。观察组不良反应发生率(4.00%)低于对照组(16.66%),差异有统计学意义(P<0.05)。结论甲泼尼龙耳后注射联合巴曲酶治疗突发性耳聋患者疗效较好,对于耳鸣症状改善较为明显且不会增加不良反应,但对于全聋的患者疗效较低。Objective To investigate the efficacy of postotic methylprednisolone injection combined with atroxobin in the treatment of sudden deafness.Methods Ninety-eight patients with sudden deafness in the First Affiliated Hospital of Henan University of Science and Technology from January 2017 to January 2019 were divided into observation group(n=50)and control group(n=48)by random number table,and were further divided into three subgroups according to different hearing impairment levels,observation group:group A(n=15),group B(n=20),group C(n=15);control group:group A(n=15),group B(n=20),group C(n=13).The control group received batroxobin and conventional treatment.The observation group received methylprednisolone sodium succinate for local postotic methylprednisolone injection on the basis of the control group.The clinical efficacy;the treatment effect,tinnitus symptoms and complications of the two groups of patients with different degrees of hearing impairment after 2 weeks were compared.Results After treatment,the total effective rate of the observation group(82.00%)was higher than that of the control group(56.28%),P<0.05.The total effective rate of observation group A was 93.33%,the total effective rate of group B(90.00%)was higher than that of control group A(60.00%)and group B(60.00%),the difference was statistically significant(P<0.05).The total effective rate of tinnitus treatment in the observation group(86.00%)was higher than that in the control group(60.25%),the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group(4.00%)was lower than that in the control group(16.66%),P<0.05.Conclusions Postotic methylprednisolone injection combined with atroxobin is effective in patients with sudden deafness,especially more obvious for tinnitus symptoms without new adverse reactions,but it is less effective for patients with total deafness.
