摘 要:目的探究肾移植术中实施小剂量多巴胺静脉泵注对维持患者血流动力学平稳的临床效果。方法方便选取该院2017年1月—2020年9月收治患者100例,每组50例,前者实施常规静吸复合全麻,后者实施常规静吸复合全麻合并小剂量多巴胺持续静脉泵注,比较两组患者情况。结果移植肾动脉、静脉开放前,两组的平均动脉压、心率、中心静脉压的对比差异无统计学意义(P>0.05);开放后5、30 min,研究组的平均动脉压(125±13)mmHg、(130±19)mmHg、心率(101±11)次/min、(103±13)次/min、中心静脉压(8.5±1.32)mmHg、(6.6±1.7)mmHg的变化均优于参照组,差异有统计学意义(t=14.658、10.426、3.636、6.022、19.328、8.237,P<0.05);研究组平均出尿时间(7.2±5.5)min短于参照组,且研究组1 h总尿量(154.7±83.6)mL,6 h总尿量(3279.9±1823.5)mL高于参照组,差异有统计学意义(t=4.265、7.612、3.415,P<0.05)。结论在肾移植术的过程中持续的泵注小剂量多巴胺,有利于移植肾功能的恢复。Objective To explore the clinical effect of low-dose dopamine intravenous pump in renal transplantation on maintaining stable hemodynamics in patients.Methods Convenient select 100 patients were admitted to the hospital from January 2017 to September 2020,50 cases in each group.The former received conventional intravenous inhalation combined general anesthesia,and the latter received conventional intravenous combined general anesthesia combined with low-dose dopamine continuous intravenous pump injection.Compared the situation of the two groups of patients.Results Before transplantation of renal artery and vein opening,there was no significant difference in the mean arterial pressure,heart rate,and central venous pressure between the two groups(P>0.05);5 and 30 minutes after opening,the average arterial pressure of the study group(125±13)mmHg,(130±19)mmHg,heart rate(101±11)beats/min,(103±13)beats/min,central venous pressure(8.5±1.32)mmHg,(6.6±1.7)mmHg changes are better than in the reference group,the difference was statistically significant(t=14.658,10.426,3.636,6.022,19.328,8.237,P<0.05);the average urination time of the study group(7.2±5.5)min was shorter than that of the reference group,and the study The 1-hour total urine output of the group(154.7±83.6)mL and the 6-hour total urine output(3279.9±1823.5)mL were higher than the reference group,and the difference was statistically significant(t=4.265,7.612,3.415,P<0.05).Conclusion Continuous pumping of small doses of dopamine during kidney transplantation is beneficial to the recovery of transplanted kidney function.
