类风湿关节炎中医诊疗方案比较研究

摘　　要：目的探讨类风湿关节炎(RA)中医联合诊疗方案的应用效果。方法选取2017年8月~2019年12月九江市第一人民医院风湿免疫科收治的120例RA患者作为研究对象,按照随机数字表法分为西药组与联合组,每组各60例。西药组采用西医治疗方案,联合组采用中医联合诊疗方案。比较两组临床疗效、治疗前后症状及体征改善情况、实验室指标及不良反应发生情况。结果联合组治疗总有效率高于西药组,差异有统计学意义(P<0.05)。联合组治疗后关节疼痛评分、关节压痛指数、关节肿胀指数低于西药组,晨僵时间短于西药组,差异有统计学意义(P<0.05)。联合组关节功能优于西药组,差异有统计学意义(P<0.05)。联合组细胞沉降率及C反应蛋白水平均低于西药组,差异有统计学意义(P<0.05)。两组患者治疗期间均未发生明显不良反应。结论采用中医联合诊疗方案治疗RA疗效确切,患者关节功能恢复较好,可促进预后恢复,值得临床推广。Objective To explore the application effect of combined traditional Chinese medicine treatment of rheumatoid arthritis(RA).Methods A total of 120 RA patients admitted to the Department of Rheumatology and Immunology of Jiujiang First People′s Hospital from August 2017 to December 2019 were selected as the research subjects.According to random number table method,they were divided into western medicine group and combined group,with 60 cases in each group.The western medicine group was treated with western medicine treatment plan,while the combination group was treated with Chinese medicine treatment plan.Clinical efficacy,improvement of symptoms and signs before and after treatment,laboratory indicators and the incidence of ADR were compared between the two groups.Results The total effective rate of combined group was higher than that of western medicine group,and the difference was statistically significant(P<0.05).After treatment,the joint pain score,joint tenderness index and joint swelling index of the combined group were lower than those of the western medicine group,and the morning stiffness time was shorter than that of the western medicine group,the differences were statistically significant(P<0.05).Joint function of the combined group was better than that of the western medicine group,and the difference was statistically significant(P<0.05).The cell sedimentation rate and C-reactive protein level in the combined group were lower than those in the western medicine group,the differences were statistically significant(P<0.05).No obvious adverse reactions occurred in two groups during treatment.Conclusion The combined treatment of traditional Chinese medicine has a definite effect on RA,and the patients have a good recovery of joint function,which can promote the recovery of prognosis,and is worthy of clinical promotion.
