摘　　要：目的探讨可视穿刺器直视下一步法建立腹膜后腔技术的临床应用价值。方法收集80例需手术治疗的上尿路疾病且行后腹腔镜手术患者,分为研究组和对照组,每组40例。研究组采用可视穿刺器直视一步法建立腹膜后腔技术,对照组采用自制球囊扩张建立腹膜后腔技术。比较2组患者腹膜后腔建立(全部置入3个套管)完成时间及不良事件(通道漏气、通道出血、脏器损伤、第一条通道建立时扩张过深或过浅)的发生率。结果与对照组相比,研究组建立腹膜后腔完成时间较短(P<0.01),且不良事件发生率低于对照组(P<0.05)。结论可视穿刺器直视一步法建立腹膜后腔技术可在直视下简单、快速、安全、有效地建立腹膜后腔,具有较高的临床应用价值。Objective To evaluate the clinical application value of visual puncture device to establish the retroperitoneal cavity technique by one-step method.Methods Eighty patients undergoing retroperitoneal laparoscopic surgery were recruited and divided into the study(n=40)and control groups(n=40).All patients had upper urinary tract diseases which were needed surgery treatment.In the study group,retroperitoneal cavity technique was established by one-step visual puncture method.In the control group,retroperitoneal cavity technique was established by self-made balloon dilation method.The time to complete the establishment of retroperitoneal cavity(all three cannulas were inserted)and the incidence of adverse events(air leakage,bleeding of the observation channel,organ damage,too deep or too shallow expansion of the first channel)were statistically compared between two groups.Results In the study group,the time to complete the establishment of retroperitoneal cavity was significantly shorter(P<0.01)and the incidence of adverse events was remarkably lower(P<0.05)compared with those in the control group.Conclusion The onestep visual puncture device is a simple,rapid,safe and effective method to establish the retroperitoneal cavity technique,which is worthy of clinical application.
