血清VEGF及VCAM-1水平与老年股骨颈骨折患者术后延迟愈合的相关性

摘　　要：目的探讨老年股骨颈骨折术后延迟愈合患者血清血管内皮生长因子(VEGF)及血管细胞黏附分子-1(VCAM-1)表达与骨折术后延迟愈合的关系。方法抽取2017年2月至2019年11月于开封市中心医院接受手术治疗的139例老年股骨颈骨折患者作为骨折组,并收集同期于医院接受健康体检的131例健康人群作为对照组。回顾性分析两组研究对象的临床资料。检测并比较两组血清VEGF及VCAM-1水平。根据骨折组患者愈合情况将其分为延迟愈合组与正常愈合组。比较、分析延迟愈合组与正常愈合组术前、术后4周、术后8周、术后12周VEGF及VCAM-1水平。采用二元Logistic回归分析检验患者术前VEGF及VCAM-1水平对术后延迟愈合的影响。绘制受试者工作特征曲线(ROC)检验各指标预测老年股骨颈骨折患者术后延迟愈合风险的价值。结果骨折组血清VEGF水平低于对照组,VCAM-1水平高于对照组(P<0.05)。骨折组139例患者中延迟愈23例。延迟组及正常愈合组术后各时点VEGF水平均较术前升高,VCAM-1水平较术前降低,且延迟愈合组术后各时点VEGF水平均低于正常愈合组,VCAM-1水平高于正常愈合组,差异有统计学意义(P<0.05)。二元Logistic回归分析结果显示,术前血清VEGF低表达及VCAM-1过表达是老年股骨颈骨折患者术后延迟愈合的影响因素(OR>1,P<0.05)。结论血清VEGF及VCAM-1表达异常可能是老年股骨颈骨折术后延迟愈合的影响因素,临床可通过检测患者术前血清VEGF及VCAM-1水平预测其术后延迟愈合风险,为术后制定合理化康复方案提供指导。Objective To investigate relationship of the expression of serum vascular endothelial growth factor(VEGF)and vascular cell adhesion molecule-1(VCAM-1)with postoperative delayed union in elderly patients with femoral neck fracture.Methods A total of 139 patients with senile femoral neck fracture treated in Kaifeng Central Hospital from February 2017 to November 2019 were selected as fracture group.And 131 healthy people who underwent physical examination in the hospital during the same period were selected as control group.The clinical data of the two groups were retrospectively analyzed.The serum VEGF and VCAM-1 levels in two groups were detected and compared.And the patients in fracture group were divided into delayed union group and normal union group according to the union status.The levels of VEGF and VCAM-1 were compared between delayed union group and normal union group before surgery,4 weeks after surgery,8 weeks after surgery,and 12 weeks after surgery.The bivariate Logistic regression analysis was used to examine the effect of preoperative levels of VEGF and VCAM-1 on delayed union of patients.The receiver operative characteristic(ROC)curve was drawn to test the value of each index in predicting postoperative delayed union in senile patients with femoral neck fracture.Results The serum VEGF level of fracture group was lower than that of control group,and the VCAM-1 level of fracture group was higher than that of control group(P<0.05).There were 23 cases of delayed union in 139 senile patients in fracture group.The VEGF levels of two groups at every time point after surgery were higher than those before surgery,and the VCAM-1 levels were lower than those before surgery(P<0.05);the VEGF levels in delayed union group at every time point after surgery were lower than those in normal union group,and the VCAM-1 levels in delayed union group were higher than those in normal union group(P<0.05).The results of bivariate Logistic regression analysis showed that preoperative low-expression of serum VEGF and over-ex
