薛汉荣治疗哮喘的学术经验与临证思路

摘　　要：薛汉荣从事肺系疾病临床工作已达20余年,在治疗哮喘方面积累了丰富的临床经验。本文对其学术经验作了初步概括,主要体现在以重视气阳虚弱为哮喘发病的重要内因,初期常为肺卫气阳虚弱渐及脾气、脾阳,终必伤及肾气和肾阳,认为风寒为诱发哮喘发作的主要外因,提倡痰瘀伏肺为久喘之宿根,并阐释了三因素之间的相关性,强调气阳虚弱、风寒外感、痰瘀伏肺三因素之间相互影响、相互为患。基于以上哮喘发病的三因素观,认为气阳虚弱当温补,风寒之邪当辛温宣散,痰瘀久伏当温化温通,临证时常用益气温阳护卫汤温补气阳,小青龙汤解表散寒,蠲哮汤疏利气机、涤痰行瘀,体现其善用“全程温法”以及“治痰治瘀以治气为先”的治疗原则。XUE Han-rong has been engaged in the clinical work of lung diseases for more than 20 years and has accumulated rich clinical experience in the treatment of asthma.This article makes a preliminary summary of its academic experience,which is mainly reflected in the following three aspects:the importance of the body deficiency of Qi and Yang is an important internal cause of the asthma.The initial stage is often the weakening of the lung guarding Qi and Yang,and gradually the spleen Qi and spleen Yang,will eventually damage the kidney Qi and kidney Yang;think that wind-cold is the main external cause of asthma attacks;advocate phlegm and blood stasis in the lung as the root of chronic asthma,and explain the correlation between the three factors,emphasizing the weakness of Qi and Yang,the external sensation of wind and cold,and phlegm and stasis in the lung factors influence each other and cause each other.Based on the above threefactor view of the onset of asthma,he believes that the weakness of Qi and Yang should be warming,the evil of wind and cold should be Xinwen Xuansan,the phlegm and blood stasis should be Wenhua and Wentong,and the Yiqi Wenyang Huwei decoction is often used to warm Qi and Yang,Xiaoqinglong Decoction is used to dispel cold,and Juanxiao Decoction is used to disperse Qi movement,clear phlegm and blood stasis,which reflects its good use of"whole-process warming method"and"treatment of phlegm and blood stasis prior to treatment of Qi".
