鸡尾酒关节周围注射应用于TKA术后疗效分析

摘　　要：目的:探讨全膝关节置换术后使用鸡尾酒对患者术后缓解疼痛及改善功能的效果。方法:选取符合标准的60例膝骨性关节炎患者,并随机分为注射组和常规组,每组各有30例。注射组在植入假体前将配置好的20ml鸡尾酒药液注入膝关节后囊,在假体植入后向脂肪垫、股四头肌腱、内外侧副韧带、鹅足、前方关节囊及关节腔等处注射30ml鸡尾酒药液。常规组不注射。在手术后观察期间施以同样的镇痛方式。观察2组在术后的6h、12h、24h、48h、72h于静止情况下和术后的24h、48h、72h于活动后的VAS评分,同时监测膝关节在术后24h、72h、168h的最大活动度和不良反应出现人数以及主动要求止痛药人数。结果:术后静息状态下6h、12h、24h、48h、72h时段和术后运动状态下24h、48h、72h时段,注射组VAS评分低于常规组(P<0.05);在24h、72h、120h时段,注射组的膝关节最大活动度高于常规组(P<0.05);注射组主动要求止痛药人数显著低于常规组(P<0.05)。结论:全膝关节置换术后采用鸡尾酒镇痛效果优良,能在早期加速提高患者膝关节功能,安全有效。Objective:To investigate the effect of using cocktail after total knee arthroplasty on postoperative pain relief and function improvement.Methods:Select 60 patients with knee osteoarthritis who meet the criteria and randomly divide them into injection group and routine group,with 30 cases in each group.In the injection group,the configured 20ml cocktail solution was injected into the posterior capsule of the knee joint before implantation of the prosthesis.After implantation of the prosthesis,it was applied to the fat pad,quadriceps tendon,medial and lateral collateral ligaments,goose foot,anterior joint capsule and joints.Inject 30ml of cocktail liquid into the cavity.No injection in the routine group.The same analgesia was applied during the observation period after the operation.Observe the VAS scores of the two groups at 6h,12h,24h,48h,72h postoperatively under static conditions and 24h,48h,72h postoperatively after activity,and monitor the maximum knee joints at 24h,72h,168h postoperatively The number of activities and adverse reactions,and the number of people actively requesting painkillers.Results:The VAS score of the injection group was lower than that of the conventional group(P<0.05)at 6h,12h,24h,48h,72h in resting state after operation and 24h,48h,72h in postoperative exercise state(P<0.05);at 24h,72h,In the 120h period,the maximum range of motion of the knee joint in the injection group was higher than that in the conventional group(P<0.05);the number of people actively requesting painkillers in the injection group was significantly lower than the conventional group(P<0.05).Conclusion:Cocktail wine after total knee arthroplasty has an excellent analgesic effect,can accelerate the improvement of knee joint function in the early stage,and is safe and effective.
