摘 要:目的探讨雾化吸入布地奈德治疗前后婴幼儿患者口咽部菌群的变化情况,以减少不良反应的发生。方法研究对象为2016年1月至12月就诊于山西省汾阳医院耳鼻咽喉科予以雾化吸入布地奈德治疗的40例婴幼儿患者,收集治疗前、治疗5 d及10 d患者的咽拭子标本行细菌培养,统计雾化治疗前后患者口咽部相关不良反应发生率及口咽部菌群变化情况。采用SPSS 19.0进行统计学分析。结果雾化治疗5 d与治疗10 d后的口腔溃疡、呕吐、腹泻、声音嘶哑、口角炎不良反应发生率比较,差异无统计学意义(P>0.05);草绿色链球菌治疗前后检出率,差异有统计学意义(P<0.05);肠球菌、金黄色葡萄球菌、黄色奈色菌、流感嗜血杆菌口咽部菌群治疗前后检出率,差异无统计学意义(P>0.05)。结论婴幼儿患者雾化吸入布地奈德可导致口咽部菌群多样性变化,可引起口咽部相关不良反应的发生,因此,临床治疗需维持口咽部菌群平衡,以减少不良反应的发生,提供临床治疗效果。Objective To investigate the alterations in oropharyngeal flora of infants and young children before and after treatment with aerosolized budesonide,so as to reduce adverse reactions.Methods Included in this study were 40 infants and young children who were treated with aerosolized budesonide in the Department of Otolaryngology,Fenyang Hospital in Shanxi between January 2016 and December 2016.Throat swabs of the patients were collected before and at 5 and 10 days after treatment for bacteria cultured.The incidence of oropharyngeal adverse reactions and changes in oropharyngeal flora before and after aerosol inhalation were recorded.SPSS19.0 was used for the statistical analysis.Results There were no significant differences in the incidence of oral ulcer,vomiting,diarrhea,hoarseness,and angular cheilitis between 5 and 10 days of the aerosol inhalation(P>0.05).There was a significant difference in the detection yield of Streptococcus viridans before and after treatment(P<0.05),but not in those of Enterococcus,Staphylococcus aureus,Neisseria aureus,Haemophilus influenzae in the oropharyngeal flora before and after treatment(P>0.05).Conclusion Inhalation of budesonide aerosol in infants and young children can cause altered diversity of oropharyngeal flora and adverse reactions related to the oropharynx.Therefore,clinical treatment should take into consideration of maintaining the balance of oropharyngeal flora,so as to reduce adverse reactions and improve the therapeutic efficacy.
