摘 要:目的:通过观察肺炎支原体(MP)感染小鼠肺、肠组织中肺表面活性蛋白A(SP-A)、可溶性B7-H3(sB7-H3)的表达,探讨MP诱导的小鼠肺损伤的机制及中药的干预作用。方法:采用MP菌液滴鼻的方法建立MP诱导小鼠感染模型。将造模成功的50只小鼠分为正常组、模型组、中药组(清支止咳汤)、西药组(阿奇霉素)和中西结合组(清支止咳汤+阿奇霉素),每组10只,清支止咳汤连续干预7d,阿奇霉素连续干预3d,各组均于第8天取材,采用免疫组化和RT-PCR的方法测定肺和肠组织中的SP-A、sB7-H3蛋白及mRNA的表达。结果:与正常组比较,模型组肺组织可见明显肺间质炎症;肠组织可见炎症细胞浸润。模型组中肺组织SP-A表达显著低于正常组(P<0.01);肺、肠组织sB7-H3及肠组织SP-A的表达显著高于正常组(P<0.01,P<0.05)。与模型组比较,各治疗组肺组织SP-A的表达均显著升高(P<0.01,P<0.05),sB7-H3表达显著降低(P<0.05);中药组和中西结合组肠组织SP-A、sB7-H3的表达显著下调(P<0.01,P<0.05),且中西结合组肺、肠组织中SP-A及肠组织中sB7-H3表达改善优于西药组(P<0.01,P<0.05)。结论:清支止咳汤联合阿奇霉素能有效促进肺组织中SP-A的表达,抑制肺、肠组织中sB7-H3及肠组织SP-A的表达,改善MP感染小鼠的肺、肠组织损伤。Objective:To explore the mechanism of lung injury in the mice infected by mycoplasmal pneumonia and provide experimental evidence for clinical treatment by observing the changes of surfactant protein A(SP-A)and B7 homolog 3 protein(sB7-H3)expressions in the lung and the intestines of the mice.The mouse model was established using the MP bacterial droplet nasal method.Fifty female BALB/c mice were randomly divided into 5 groups:control,model,Qingzhi Zhike Decoction group,western medicine group and Chinese and Western combination group,10 mice in each group.Qingzhi Zhike Decoction was administered by oral gavage every day for seven days,and azithromycin was administered for three days,the lung and intestines tissues were collected after 7 days to observe the inflammation of the tissues.The levels of SP-A and sB7-H3 expressions in lung and intestines tissues were assessed by immunohistochemistry staining and RT-PCR.Results:The pathological results showed that compared with the control group,the lung tissue showed obvious pulmonary interstitial inflammation;the intestinal tissue showed inflammatory cell infiltration.The expressions of SP-A in the lung tissue were significantly lower(P<0.01),while the s B7-H3 in lung and intestine tissues and SP-A in the intestine tissue were significantly higher in the model group than the control group(P<0.01,P<0.05).Compared with the model group,the expression of SP-A in the lung tissue in Qingzhi Zhike Decoction group,western medicine group and Chinese and Western combination group were significantly increased(P<0.01,P<0.05),and s B7-H3 in lung tissue were decreased(P<0.05);while the expression of SP-A and sB7-H3 in the intestinal tissue in Qingzhi Zhike Decoction group and Chinese and Western medicine group were significantly decreased(P<0.01,P<0.05).The improvement of SP-A in the lung and intestine tissues and sB7-H3 in the intestinal tissue in Chinese and Western medicine group were better than those in the western medicine group(P<0.01,P<0.05).Conclusion:Qingzhi Zhike Decoctio
