摘　　要："肺主行水,通调水道"指肺气的宣发肃降是人体津液运行的主要动力。气道黏液在中医上归属于"津",气道黏液高分泌归属于"痰"。肺通调水道功能障碍引起的气道黏液高分泌是促进慢性阻塞性肺疾病(COPD)发生发展的重要病理因素,其主要机制为炎症反应、氧化应激、蛋白酶/抗蛋白酶失衡导致的杯状细胞化生增加及黏膜纤毛清除机制受损。文章基于"肺通调水道"理论阐述了气道黏液高分泌在COPD发生发展中的意义,提出改善气道黏液高分泌为治疗COPD的主要靶点,为COPD的治疗及进一步研究提供新思路。‘The lung dominates the body fluid flow,unclogging and regulating the channels’means that the promotion and lowering of lung qi is the main driving force of body fluid.Airway mucus is attributed to‘jin’in traditional Chinese medicine(TCM),and airway mucus hypersecretion is attributed to‘phlegm’.Airway mucus hypersecretion caused by the dysfunction of lung in unclogging and regulating the fluid flow channels is an important pathological factor promoting the development of chronic obstructive pulmonary disease(COPD),and its main mechanism is inflammation,oxidative stress,increased goblet cell metaplasia caused by protease/antiprotease imbalance,and impaired mucociliary clearance mechanism.In this paper,the significance of airway mucus hypersecretion in the occurrence and development of COPD was expounded based on the theory of‘lung unclogging and regulating the fluid flow channels’,and the improvement of airway mucus hypersecretion was proposed as the main target for the treatment of COPD,providing a new idea for the treatment and further research of COPD.
