摘 要:目的:观察MRI联合超声成像在类风湿性腕关节炎患者关节活动度评价中的价值。方法:选取2019年1~10月收治的类风湿性腕关节炎患者50例,依据腕关节DAS28评分结果分为极难活动度组(12例)、低活动度组(16例)、中活动度组(12例)、高活动度组(10例)。以Spearman秩相关性法观察MRI成像评分、超声成像评分与DAS28评分相关性,并采用受试者工作特征曲线(ROC曲线)法分析MRI成像评分、超声成像评分在评价关节活动度分级中的价值。结果:极难活动度组、低活动度组、中活动度组、高活动度组MRI成像评分、超声成像评分、RF、CRP、抗CCP抗体比较,差异具有统计学意义(P<0.05)。MRI成像评分和超声成像评分均与DAS28评分、RF、CRP、抗CCP抗体呈正相关关系(P<0.001)。MRI联合超声成像评分鉴别不同活动度的准确率、敏感性,特异性均在80%以上。结论:MRI成像评分、超声成像评分与类风湿性腕关节炎患者的关节活动度存在相关性,且MRI联合超声成像在评价类风湿性腕关节炎患者关节活动度程度中具有较高临床价值。Objective: To observe the value of MRI combined with ultrasonography in the evaluation of activity of patients with rheumatoid carpal arthritis. Methods: A total of 50 cases of rheumatoid arthritis of the wrist were selected as the study objects. The patients were divided into four groups: extremely difficult activity group(12 cases), low activity group(16 cases), medium activity group(12 cases), and high activity group(10 cases). The correlation between MRI score, ultrasound score and arthritis activity were observed by Spearman rank correlation method. Receiver operating characteristic curve(ROC) method was used to analyze the value of MRI and ultrasound imaging scores in the evaluation of range of arthritis activity. Results: MRI scores, ultrasound scores,RF, CRP, anti-CCP antibody of extremely difficult mobility group, low mobility group, moderate mobility group and high mobility group were compared, the differences were statistically significant(P<0.05). MRI scores and ultrasound imaging score were positively correlated with DAS28, RF, CRP and anti-CCP antibody(P<0.001).The accuracy and sensitivity of MRI combined with ultrasonic imaging score in differentiating activity were all above 80%. Conclusion: There was correlation between MRI imaging score, ultrasound imaging score and joint range of motion of patients with rheumatoid wrist arthritis, and MRI combined with ultrasound imaging has high clinical value in evaluating the joint range of motion of patients with rheumatoid wrist arthritis.
