血镁水平与脊柱结核患者术后住院时间和机械通气时间的相关性分析

摘　　要：目的观察重症病房(ICU)脊柱结核术后患者血镁水平与其术后机械通气时间和术后住院时间之间的关系.方法本院重症病房收治的脊柱结核术后患者73例,记录患者入住本科时的血镁浓度及其他参数和患者术后机械通气时间及术后住院时间,观察比较并进行相关性分析.结果73例入选患者中低血镁(<0.7 mmol/L)患者24例,正常血镁(0.7~1.2 mmol/L)患者49例,未见明显高血镁患者,低血镁患者占比32.88%.低血镁组和正常血镁组患者的基础疾病组成比较,高血压病和糖尿病差异无统计学意义(P>0.05).低血镁组患者术后机械通气时间高于正常血镁组,同样低血镁组患者术后住院时间也高于正常血镁组,差异有统计学意义(P<0.05).两组急性生理学与慢性健康状况评分系统Ⅱ(APACHEII)差异具有统计学意义(P<0.05).两组患者血钙、血磷、血钾、血钠、血C-反应蛋白、血白蛋白、血乳酸水平及ICU住院时间比较,差异无统计学意义(P>0.05).多因素Logistic回归分析发现,脊柱结核术后患者6 h机械通气时间和14 d住院时间与血镁呈负相关(P<0.05),APACHEII评分与血镁呈正相关(P<0.05),而与血乳酸、血CRP、血白蛋白及ICU住院时间无相关(P>0.05).结论低血镁水平与重症病房脊柱结核术后患者术后机械通气时间和术后住院时间有关,适当纠正血镁水平可能会改善患者预后.ObjectiveTo observe the relationship between the level of serum magnesium and the time of mechanical ventilation and hospialization in patients with spinal tuberculosis after operation in ICU. Methods 73 patients with spinal tuberculosis adnmitted to our intensive care unit, the senum magnesium level and other parameters were recorded when the patients entered our department. The time of mechanical ventilation in our center and the time of hospitalization were recorded. The orrelation between the serum magnesium level and other parameters and the time of postoperative mechanical ventilation and the time of hospitalizaion were analyzed. Results There were 24 patients with hypormagnesemia ( <0.7 mmol/L ) and 49 patients with normal serum magnesium ( 0.7~1.2 mmol/L ) . No significant hypermagne semnia was found. Hypomagnesemia accounted for 32.88% There was no signifcant diference in hypertension and diabetes between hypomagnesenia group and hypermagnesemia group ( P>0.05 ) .The postoperative mechanical venilation time of hyomagesemia group was higher than that of normal blood magnesium group, and the postoperative hospitalization time of hypomagnesemia group was abo higher than that of normal blood magneium goup, with stsical diflference ( P<0.05 ) . At the same time, it was found that there was a signifcant diference between the two groups in Acute Plhysiology and Chronic Health Evaluation II score ( P<005 ) .However, there was no significant diference between the two groups in seum calcium, senum phosphorus, serum potssium, serum sodium, serum C- reactive protein, serum albumin, serum lactate level and ICU hopitaliz ation time ( P>0.05 ),Mulivariate logistic rgeresion analysis showed that the 6- hour mechanical ventilation time and 14 day hopialization time were :negatively correlated with serum magnesium ( P<0.05),APACHE II score was positively correlated with serum magnesium ( P<0.05 ),but not with blood lactate, CRP and albumin ( P>0.05 ) .Conclusion Hyomagnscemia is related to the time of mechanical venilatio
