摘  要:目的探讨miR-374a在糖尿病肾病(DN)患者外周血中的表达及对炎症的调控机制。方法选择2017年1月-2020年1月河北省保定市第一中心医院收治的44例T2D患者为研究对象(T2D组)。根据是否并发DN,将患者分为无DN组(n=27)及DN组(n=17)。根据疾病严重程度,将DN组患者分为大量蛋白尿组(n=10)和微量蛋白尿组(n=7)。选择同期在本院治疗的13例合并视网膜病变且白蛋白尿检查正常的T2D患者为阳性对照(阳性对照组),以及同期在我院行健康体检的24名非糖尿病健康人为阴性对照组,比较各组血清miR-374a表达水平,检测并分析DN组和无DN组多形核白细胞(PMN)滚动速度和黏附性、血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和细胞间黏附分子-1(ICAM-1)水平。结果T2D组与阴性对照组的血清miR-374a水平分别为(1.28±0.24)、(1.00±0.17),差异无统计学意义(P>0.05)。与无DN组相比,DN组血清中miR-374a水平较低(P<0.01),而阳性对照组的miR-374a水平高于DN组(P<0.01)。大量蛋白尿组miR-374a水平低于无DN组(P<0.01)及微量蛋白尿组(P<0.05)。Logistic多因素分析结果显示,超敏C-反应蛋白(hs-CRP)、空腹C肽及血清总胆固醇是miR-374a表达水平的独立影响因素(P<0.05)。与无DN组相比,DN组的PMN滚动速度较低(P<0.05),黏附性较高(P<0.01)。miR-374a与PMN滚动速度呈正相关(r=0.625;P<0.01),而与PMN黏附性呈负相关(r=-0.457;P<0.05)。与无DN组比较,DN组TNF-α、IL-6、ICAM-1水平较高(P<0.05)。miR-374a与TNF-α(r=-0.592,P<0.05)、IL-6(r=-0.491,P<0.05)和ICAM-1水平(r=-0.687,P<0.05)呈负相关。结论DN患者血清中miR-374a高表达,可能通过影响机体炎症反应参与DN的发生和发展。Objective To investigate the expression of miR-374a in peripheral blood of patients with diabetic nephropathy(DN)and its regulation mechanism on inflammation.Methods Between January 2017 and January 2020,44 patients with T2D in Baoding first Central Hospital of Hebei Province were selected as the research objects(T2D group).The patients were divided into non DN group(n=27)and DN group(n=17).According to the severity of the disease,patients in DN group were divided into massive proteinuria group(n=10)and microalbuminuria group(n=7).The 13 T2D patients with retinopathy and normal albuminuria that were treated in hospital during the same period were selected as the positive control group,and 24 non-diabetic healthy people who underwent a physical examination in our hospital during the same period were selected as the negative control group.The expression levels of serum miR-374a in each group were detected and analyzed.The rolling speed and adhesion of polymorphonuclear leukocytes(PMN),serum tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6)and intercellular adhesion molecule-1(ICAM-1)were detected in the DN group and the non-DN group.Results The serum miR-374a levels of the T2D group and the negative control group were(1.28±0.24)and(1.00±0.17)respectively,and the difference was not statistically significant(P>0.05).Compared with the non-DN group,the level of miR-374a in the serum of the DN group was lower(P<0.01),while the level of miR-374a in the positive control group was higher than that of the DN group(P<0.01).The level of miR-374a in the massive proteinuria group was lower than that of the non-DN group(P<0.01)and the microalbuminuria group(P<0.05).Logistic multivariate analysis showed that high-sensitivity C-reactive protein(hs-CRP),fasting Cpeptide and serum total cholesterol were independent factors influencing the expression level of miR-374a(P<0.05).Compared with the non-DN group,the PMN rolling speed of the DN group was lower(P<0.05),and the adhesion was higher(P<0.01).miR-374a was positively corr
