摘  要:目的探讨烟草依赖者静息态功能磁共振(rs-fMRI)下大脑奖赏网络是否会发生变化,并分析吸烟依赖程度与奖赏网络的相关性。方法招募2019年7月—2020年2月于我院保健科和体检中心查体的男性烟草依赖者28例(吸烟组),同时招募与吸烟组相匹配的从未吸烟者27例(对照组)。应用GE3.0 T磁共振对所有受试者行脑部rs-fMRI扫描,对图像进行静息态处理,比较两组受试者大脑双侧奖赏网络功能连接(FC)强度,以及大脑双侧奖赏网络FC强度与尼古丁依赖检测(FTND)分值的相关性。结果两组比较吸烟组奖赏网络FC降低的脑区主要有右侧后扣带回、右侧梭状回、右侧枕中回、右侧小脑前叶和左侧岛脑;增高的脑区主要在左侧额上回、右侧颞叶(P<0.01)。奖赏网络FC强度与FTND分值呈正相关的脑区主要有海马旁回、右岛脑;呈负相关脑区主要在左颞中回和左额下回。结论烟草依赖者的奖赏网络发生了变化,而且这种变化的奖赏网络模式与烟草依赖程度有关。Objective To investigate whether there are changes in brain reward network on resting-state functional magnetic resonance imaging(rs-fMRI)in individuals with tobacco dependence,as well as the correlation between the degree of tobacco dependence and the reward network.Methods A total of 28 male subjects with tobacco dependence who underwent physical examination in Department of Healthcare and Physical Examination Center in our hospital from July 2019 to February 2020 were recruited as smoking group,and 27 non-smokers,matched with the smoking group,were recruited as control group.GE3.0 T magnetic resonance imaging was used to perform brain rs-fMRI scan for all subjects,and the images were processed in resting state.The intensity of functional connectivity(FC)of the bilateral reward networks was compared between the two groups,and the correlation between the intensity of FC of the bilateral reward networks and Fagerstrom Test for Nicotine Dependence(FTND)score was analyzed.Results In the smoking group,the brain regions with reduced FC of the reward network included the right posterior cingulate gyrus,the right fusiform gyrus,the right middle occipital gyrus,the right anterior lobe of cerebellum,and the left insula,and the brain regions with enhanced FC were mainly the left superior frontal gyrus and the right temporal lobe(P<0.01).Positive correlation between the intensity of FC of the reward network and FTND score was observed in the parahippocampal gyrus and the right insula,while negative correlation was observed in the left middle temporal gyrus and the left inferior frontal gyrus.Conclusion There are changes in the reward network of individuals with tobacco dependence and the pattern of such changes is associated with the degree of tobacco dependence.
