儿童单侧耳聋特征及高危因素临床研究

摘　　要：目的探讨儿童单侧耳聋的临床特征,分析与儿童单侧耳聋发生相关的高危因素,为儿童单侧耳聋的防治提供依据。方法选择2019年1月~2021年1月在本地区妇幼保健机构进行儿童听力筛查未通过或家长近期发现听力可疑异常,到本院进行听力诊断为单侧耳聋的儿童36名作为病例组,选择同期我院听力诊断为正常的50名儿童作为对照组,进行病例对照研究。利用Logistic回归分析相关因素与疾病的关系。结果单侧耳聋(single-side deafness,SSD)组具有妊娠期疾病史(n=4,11.11%)的比例大于对照组(n=2,2.0%)(P=0.028);SSD组出生后患支原体肺炎超过10天(n=7,19.4%)的比例高于对照组(n=1,2%()P=0.006);SSD组对外界声音的反应为灵敏(n=24,66.7%)的比例小于对照组(n=44,88.0%()P=0.007),以上差异均具有统计学意义(P<0.05)。多因素分析提示,儿童出生后患支原体肺炎超过10天(P=0.006;OR=11.83;95%CI:1.3~101.03)使儿童患单侧耳聋的风险显著增加。结论儿童出生后患肺炎超过10天是儿童单侧耳聋的危险因素。Objective To explore the clinical characteristics of single-sided deafness(SSD)in children,and to analyze its high risk factors so as to provide the basis for the prevention and treatment of single-sided deafness in children.Methods From January 2019 to January 2021,36 children with single-sided deafness were selected as the group,and 50 children with normal hearing were selected as the control group.Logistic regression was used to analyze the relationship between related factors and diseases.Results The proportion of SSD group with gestational disease history(n=4,11.11%)was higher than that of the control group(n=2,2.0%);the proportion of SSD group with pneumonia more than 10 days after birth(n=7,19.4%)was higher than that of the control group(n=1,2%);the proportion of SSD group with sensitive response to external sound(n=24,66.7%)was lower than that of the control group(n=44,88.0%),The above differences were statistically significant(P<0.05).Multivariate analysis showed that pneumonia more than 10 days after birth(P=0.006;or=11.83;95%CI:1.39-101.03)significantly increased the risk of unilateral deafness.Conclusion Pneumonia more than 10 days after birth is a risk factor for single-sided deafness in children.
