保留髓内钉附加锁定钢板微创治疗股骨骨折髓内钉固定术后骨不连的效果研究

摘　　要：目的针对股骨骨折髓内钉固定术后骨不连患者在临床中采用保留髓内钉附加锁定钢板微创治疗后的效果进行观察。方法选取2018年10月至2019年10月本院收治的股骨骨折髓内钉固定术后骨不连患者52例,将其采用随机数字方法分为试验组与常规组,每组各26例,试验组给予保留髓内钉附加锁定钢板微创治疗,常规组给予髓内钉更换治疗,比较两种治疗方案对患者骨性愈合优良率、并发症情况以及满意度等的影响。结果常规组的骨性愈合优良率为76.92%,患者满意度分别为84.62%,试验组的骨性愈合优良率为96.15%,患者满意度分别为100.00%,两组比较,差异有统计学意义(P<0.05);两组患者治疗后的感染、内固定松动和断裂等并发症比较,差异无统计学意义(P>0.05)。结论保留髓内钉附加锁定钢板微创治疗针对股骨骨折髓内钉固定术后骨不连患者的临床效果及患者满意度均具有显著的作用,可进行临床推广和应用。Objective To observe and analyze the effect of minimally invasive treatment with retaining intramedullary nail and attaching locking plate for bone nonunion of femoral fractures after intramedullary nail fixation.Methods A total of 52 patients with bone nonunion of femoral fractures after intramedullary nail fixation were selected from October 2018 to October 2019 in our hospital.The patients were divided into experimental group and conventional group by random number method,with 26 cases in each group.The experimental group was given minimally invasive treatment with retaining intramedullary nail and attaching locking plate.The conventional group was given intramedullary nail replacement.The effects of the two treatment options on the clinical effects such as patients′healing excellent and good rate,complications,and satisfaction were compared between the two groups.Results The excellent and good rate of bone healing was 76.92%,and patient satisfaction in the conventional group was 84.62%.The excellent and good rate of bone healing was 96.15%,and patient satisfaction in the experimental group was 100.00%.There was a statistically significant difference between the two groups(P<0.05);the complications of infection,internal fixation loosening and rupture were compared between the two groups of patients after treatment,and there was no statistically significant difference between the groups(P>0.05).Conclusion The minimally invasive treatment with retaining intramedullary nail and attaching locking plate for bone nonunion of femoral fractures after intramedullary nail fixation has a significant effect on the clinical effects and patient satisfaction,which can be clinically promoted and applied.
