补体C3、C4对女性原发性干燥综合征的诊断价值

摘　　要：目的分析补体C3、C4对女性原发性干燥综合征(Primary Sjogren′s syndrome,pSS)的诊断价值。方法选取2012年1月至2019年8月台州医院首诊为pSS的女性患者346例为pSS组,选取同期女性健康体检者221例为对照组。比较两组实验室检测指标,用ROC曲线分析补体C3、C4对女性pSS的诊断价值。结果pSS组补体C3、C4显著低于对照组,差异有统计学意义(P<0.01)。补体C3、C4与pSS呈显著负相关(r=-0.526,P<0.001;r=-0.528,P<0.001)。补体C3诊断pSS的最佳截断值为0.92 g/L,曲线下面积(AUC)为0.825(95%CI:0.792~0.859),约登指数49%;补体C4诊断pSS的最佳截断值为0.22 g/L,AUC为0.826(95%CI:0.791~0.861),约登指数51%;补体C3联合补体C4诊断pSS的AUC为0.861(95%CI:0.831~0.891),约登指数57%。结论补体C3、C4可作为诊断女性pSS的有效血清学指标,二者联合诊断效果更佳。Objective To analyze the diagnostic value of complement C3 and C4 in primary Sjogren′s syndrome(pSS)in women.Methods A total of 346 female pSS patients first diagnosed in Taizhou Hospital from January 2012 to August 2019 were selected as the pSS group.Meanwhile,a total of 221 female healthy physical examination subjects were selected as the control group.The clinical information and laboratory test indexes were compared between the two groups,the ROC curve was used to analyze the diagnostic value of complement C3 and C4 in female pSS.Results Complement C3 and C4 in pSS group were significantly lower than those in the control group,the differences were statistically significant(P<0.01).Complement C3 and C4 were strongly negatively correlated with pSS(r=-0.526,P<0.001;r=-0.528,P<0.001).The optimal cut-off value of complement C3 for pSS diagnosis was 0.92 g/L,the area under the curve(AUC)was 0.825(95%CI:0.792-0.859),the Jorden index was 49%.The optimal cut-off value of complement C4 for pSS diagnosis was 0.22 g/L,AUC was 0.826(95%CI:0.791-0.861),the Jorden index was 51%.The AUC of complement C3 combined with C4 for pSS diagnosis was 0.861(95%CI:0.831-0.891),the Jorden index was 57%.Conclusion Complement C3 and C4 can be used as useful serological indicators in diagnosing female pSS,the combination of the two has a better diagnostic effect.
