骨瓜提取物联合华蟾素注射液治疗溶骨性骨转移癌的临床观察

摘　　要：目的回顾性研究骨瓜提取物注射液联合华蟾素注射液治疗恶性肿瘤骨转移癌的临床疗效。方法2016年3月-2018年3月,共66例溶骨性骨转移癌患者,其中34例接受骨瓜(猪四肢骨和甜瓜籽)提取物注射液联合华蟾素注射液治疗(治疗组),32例接受华蟾素注射液单药治疗(对照组)。治疗组给予骨瓜提取物注射液100 mg加入0.9%氯化钠注射液250 mL静脉滴注,每日1次,连续治疗28 d为1个疗程。对照组为华蟾素注射液20 mL加入葡萄糖注射液250 mL静脉滴注,28 d。观察2组的临床疗效,观察4周内患者KPS改善情况、比较2组治疗前后骨密度(BMD)水平、比较2组治疗前后NRS疼痛评分。结果治疗后,治疗组KPS评分显著高于对照组,2组总有效率分别为88.2%、78.1%,2组比较差异有统计学意义(P<0.05)。治疗后,2组NRS疼痛评分均有下降,同组治疗前后比较差异有统计学意义(P<0.05);BMD治疗组和对照组与治疗前比较及2组间比较差异均无统计学意义(P>0.05)。结论骨瓜提取物注射液联合华蟾素注射液治疗溶骨性骨转移癌可有效减轻患者疼痛,对改善生活质量有明显的疗效,有一定的临床应用价值。Objective To retrospectively study the clinical effect of Gugua extract injection combined with Cinobufosini injection in the treatment of bone metastases induced by malignant tumors.Methods From March 2016 to March 2018,a total of 66 patients with osteolytic bone metastases were enrolled in the study.34 of them were treated with Gugua extract injection(pig limb bones and melon seeds)combined with Cinobufacini injection(treatment group)and 32 with Cinobufacini injection alone(control group).In the treatment group,100 mg of Gugua extract injection and 250 mL of 0.9%sodium chloride injection were given intravenously once a day for 28 days as a treatment course,while in the control group,20 mL Cinobufacini injection and 250 mL glucose injection were infused intravenously also for 28 days as a course.The clinical efficacy,KPS improvement within 4 weeks,bone mineral density(BMD)level before and after treatment,and NRS pain score before and after treatment were compared between the two groups.Results After treatment,the KPS score of the treatment group was significantly higher than that of the control group,and the total effective rates of the two groups were 88.2%and 78.1%,respectively,with a statistically significant difference between the two groups(P<0.05).After treatment,the NRS pain score of the two groups decreased,and there was a significant difference before and after treatment in the same group(P<0.05),while there was no statistically significant difference between the treatment group and the control group and in comparison with before treatment(P>0.05).Conclusion Gugua extract injection combined with Cinobufacini injection can effectively reduce the pain of patients with osteolytic bone metastases,have obvious curative effect on improving the quality of life,and have certain clinical application value.
