《外台秘要方》治疗骨折文献的用药规律研究

摘　　要：本文采用中医文献整理的方法,通过梳理《外台秘要方》中骨折用药及相关文献,从病证与方药方面总结其运用特点。本文通过对《外台秘要方》治疗骨折方药进行分类汇总,共有续断、化瘀、止痛、补虚四法。续断法中外用以生地黄凉血充髓益骨、附子温肾逐风益骨,内服以填髓续骨、行血化瘀为主;化瘀法中瘀血留在骨节处,多用虻虫、牡丹皮,瘀血留于腹中,多用大黄,配伍桃仁、当归、蒲黄;止痛法有麻醉止痛、化瘀止痛、除热止痛之分;肾合骨,骨为髓之府,髓养骨,骨折会影响到肾的精气;补虚法是指治疗后期,亦注重补虚养骨。《外台秘要方》对骨折用药药少而精,并对预后护理禁忌加以阐述,对现代临床具有指导意义。This paper tentatively summarizes the characteristics of syndrome differentiation and medication in Waitai Miyao Fang by sorting out the varying medication methods for fracture treatment recorded in the book and other related literature with the literature collation method.In this paper,the prescriptions and meditation for the treatment of fractures in the Waitai Miyao Fang are classified and summarized into four types,namely the reuniting rupture method,the removing blood stasis method,the relieving pain method and the replenishing deficiency method.In the reuniting rupture method,fresh rehmannia root is externally applied to cool blood,strengthen marrow and nourish bone,and prepared common monkshood branched root is used to warm kidney,expel wind and benefit bone,while internal administration is mainly used to fill marrow and renew bone,to activate blood circulation and remove blood stasis.In the method of removing blood stasis,gadfly and tree peony root bark are commonly used if the blood stasis stays at the bone joints,while rhubarb root and rhizome is usually used,compatible with peach seed,Chinese angelica,cattail pollen,when the blood stasis is located in the abdomen.The relieving pain method can further be divided into anesthesia,removing blood stasis and removing heat.As the kidney is consistent with bones,and bones are the home of marrows,while marrows in return nourish bones,therefore fractures may affect the essence and qi of the kidney.Accordingly,the replenishing deficiency method emphasizes replenishing deficiency and nourishing bones at the later stage of the treatment.Waitai Miyao Fang is characterized as its fewer but better medicines for fractures and its expounding on taboos in prognostic nursing,having a guiding significance for modern clinical practice.
