摘  要:目的通过调节性T细胞初步探讨梅毒合并HIV感染的免疫特点。方法在男男性接触者队列中收集26例早期梅毒、18例HIV感染、18例早期梅毒合并HIV感染患者的临床资料,并选取20例健康者作为对照。流式细胞术统计分析以上四组及早期梅毒组、合并组梅毒不同分期调节性T细胞(Treg细胞)水平差异。结果HIV组和早期梅毒组Treg细胞占PBMC百分比明显高于健康组(0.80%±0.10%,0.74%±0.07%vs 0.32%±0.05%,均P<0.05);合并感染组Treg细胞所占PBMC百分比明显高于健康组(1.01%±0.09%vs 0.32%±0.05%,P<0.001)。将单纯梅毒与合并感染患者按照不同临床分期进行分组,在一期进展为二期梅毒时Treg细胞占PBMC百分比均升高,但合并感染患者上升斜率更高。结论Treg细胞占PBMC百分比在早期梅毒合并HIV感染患者中明显升高,提示梅毒合并HIV感染时,机体的免疫抑制更明显。Objective To observe the change of regulatory T cells in syphilis and HIV co-infection patients.Methods A total of 26 early syphilis patients(including primary,secondary and early latent syphilis),18 HIV patients(CD4≥350/μl,course of infection>6 months),18 early syphilis and HIV co-infection patients(CD4≥350/μl,course of infection>6 months)and 20 healthy controls were enrolled from men who have sex with men(MSM)in this study.The regulatory T cells(Tregs)were analyzed in all subjects and different stage patients co-infected with early syphilis and HIV using flow cytometry.Results The percentage of Tregs in HIV group and early syphilis group was higher than that in healthy control group(0.80%±0.10%,0.74%±0.07%vs 0.32%±0.05%,P<0.05).The percentage of Tregs in co-infection group was higher than that in healthy control group(1.01%±0.09%vs 0.32%±0.05%,P<0.001).The percentage of Tregs was increased faster in co-infection group than that in early syphilis group.Conclusion The percentage of Tregs in co-infection patients is significantly higher than that of syphilis and HIV patients,indicating that co-infection may aggravate the immunosuppression.
