规范化护理在护士对改善下肢静脉曲张患者围术期抬高患肢情况的效果分析

摘　　要：目的通过规范护士专科护理工作、提高护士对下肢静脉曲张患者围术期抬高患肢宣教率,保证患者抬高患肢等有效的治疗。方法本次研究采用随机分组方法将入选患者分为使用脚垫组(n=39)和抬高床尾组(n=108),护士在患者入院时、手术前、手术后、出院时分别进行抬高患肢等疾病相关宣教,患者住院期间通过使用脚垫和抬高床尾2种方式抬高患肢。通过标准化优质护理流程,包括自制问卷、Kolcaba舒适度状况量表、NRS疼痛评分量表等手段,对本科2019年7月至11月147例下肢静脉曲张患者(年龄范围为18~65岁)不同抬高患肢方式的舒适度、执行率以及护理宣教率等进行观察;统计两组患者住院期间并发症发生率、手术等治疗情况、患肢肿胀缓解情况,护士宣教率及患者抬高患肢执行率、患者舒适度等指标。结果从总体的宣教情况而言,7月至11月的宣教率均高于改进前4月至6月,护士的宣教率与患者的执行率呈正相关,可见规范化的护理措施对改善护士的宣教情况有意义;从第1次宣教点分析,大部分患者在入院时进行第1次宣教,宣教率为88.44%、患者抬高患肢的执行率为70.37%~100.00%、平均执行率为80.95%,症状缓解率为60.87%~75.00%、平均症状缓解率为67.69%;少数患者在其他时间点进行第1次宣教,宣教率为97.96%。结论规范化护理能有效提高护士的专科护理水平,使临床护理工作达到标准化,从而改善护士的宣教情况,提高本科的优质护理质量,保证患者治疗稳定有效的进行。Objective By regulating the nursing work of nurses,improving the teaching rate of limb elevation for patients with varicose veins of lower extremities during the perioperative period,to ensure the effective treatment of patients with varicose veins of lower extremities.Methods In this study,the selected patients were divided into using foot pads group(n=39)and raising the end of bed group(n=108)by random grouping method.Nurses carried out relevant education on diseases such as elevation of affected limb at the time of admission,before operation,after operation,and at the time of discharge.During hospitalization,patients'affected limbs were raised by using foot pads and raising the end of bed.Through standardized high-quality nursing process,including self-made questionnaire,Kolcaba comfort level scale,NRS pain score scale and other means,the comfort level,implementation rate,and nursing education rate of 147 patients with varicose veins of lower extremities aged 18-65 in our department from July to November 2019 were observed.The incidence of complications during hospitalization,surgical treatment,swelling relief of affected limbs,nurse education rate,implementation rate of patients with varicose veins of lower extremities,patient comfort level,and other indicators were counted.Results From the overall propaganda and education situation,the propaganda and education rate from July to November was higher than that from April to June.The propaganda and education rate of nurses was positively correlated with the implementation rate of patients,which showed that standardized nursing measures were significant in improving the propaganda and education situation of nurses.From the analysis of the first education points:most patients received the first education when they were admitted to hospital,the education rate was 88.44%,the implementation rate of patients raising affected limbs was 70.37%-100.00%,the average implementation rate was 80.95%,the symptom relief rate was 60.87%-75.00%,and the average symptom relief rate
