加长柄人工全髋关节置换治疗高龄股骨转子间骨折合并股骨头坏死的临床疗效分析

摘　　要：目的:探讨加长柄人工全髋关节置换术治疗高龄股骨转子间骨折合并股骨头坏死患者的临床应用效果。方法:回顾性分析攀枝花市中心医院骨科2015年1月至2018年7月期间21例高龄股骨转子间骨折合并股骨头坏死的患者行加长柄人工关节置换术中的临床效果。结果:所有患者均同期采用上述方法,术后均取得随访,平均随访时间(14.0±1.2)月;所有患者术后骨折均取得骨性愈合,内固定钢丝或线缆均无断裂或松动现象,人工关节假体位置均良好,无松动、下沉等现象;髋关节活动基本正常且较手术前明显改善;所有患者对此次治疗均满意。结论:该方案能够同时解决患者骨折及股骨头坏死问题,避免了反复多次手术给患者带来的痛苦,最大程度减轻了患者及家属的经济负担,提高患者及家属对治疗的满意程度,是临床上实用的一种重要方法。Objective:To discuss the clinical efficacy of long-stem total hip arthroplasty in the treatment of intertrochanteric fracture with femoral head necrosis in the elderly.Methods:We retrospectively analyzed clinical efficacy of long-stem total hip arthroplasty in the treatment of intertrochanteric fracture with femoral head necrosis in 21 patients in the Department of Othopedics in Panzhihua Central Hospital from January 2015 to July 2018.Results:All the patients underwent the above-mentioned surgery at the same time,and were followed up after surgery,with an average follow-up time of(14.0±1.2)months.All patients had bone healing,and there were no break or looseness in the internal fixation steel wire or cable;the position of the joint prosthesis was good without loosening or sinking;the activity of hip joint was normal and improved significantly;all patients were satisfied with this treatment.Conclusion:Long-stem total hip arthroplasty can solve the problems of fracture and necrosis of femoral head at the same time,avoid the pain of patients caused by repeated operations,minimize the financial burden of patients and their families,and improve the satisfaction of patients and their families with the treatment.So,it is an important and practical clinical method.
