血友病性关节炎手术治疗的研究进展

摘　　要：血友病是凝血因子缺乏所致的遗传性出血性疾病,而血友病性关节炎是血友病患者主要的并发症之一。血友病性关节炎具有发病早、进展快、疾病发展进程不易控制等特点,常造成关节肿胀、关节疼痛和关节挛缩畸形,最终导致关节功能严重受限。在多学科密切合作下行手术治疗可以明显改善患者的生活质量。但是目前关于血友病性关节炎的手术策略及其疗效和风险还在探索中,本文主要对血友病性关节炎的发病机制、影像学评估及关节病变分级、常用的手术方式、围手术期注意事项、近几年的术后疗效病例报道进行总结,旨在为手术治疗血友病性关节炎提供参考。Hemophilia is an inherited bleeding disease caused by the deficiency of coagulation factors,and hemophilic arthritis is one of main complications in patients with hemophilia.Due to the characteristics of early onset,rapid progress,and difficulty in controlling the disease's progression,hemophilic arthritis often causes joint swelling,pain and contracture deformity,which ultimately lead to severe joint function limitations.Surgical interventions under the multidisciplinary close cooperation can significantly improve the quality of patients’life.However,the surgical strategies,curative effect and risks about hemophilic arthritis are still under exploration.This paper summarizes the pathogenesis of hemophilic arthritis,imaging evaluation and arthropathy classification,common surgical techniques,perioperative management and postoperative case reports in recent years,aiming to provide a reference for surgical interventions in hemophilic arthritis.
