广义Epsilon波对ARVC诊断价值的研究

摘　　要：目的比较广义Epsilon波与Epsilon波诊断致心律失常性右室心肌病的敏感性,分析其诊断价值。方法收集2009年1月至2020年11月北京大学人民医院确诊致心律失常性右室心肌病患者、检索同期中国知网及Pubmed数据库中个案,比较Epsilon波与广义Epsilon波的诊断敏感性;统计广义Epsilon波各亚型检出率和导联分布情况。结果共纳入24例患者,检出Epsilon波4例,广义Epsilon波19例,其诊断敏感性依次为16.7%和79.2%,Pearsonχ^(2)=18.783,p=0.000。广义Epsilon波中各亚型:Presilon波5例(20.8%)、Topsilon波3例(12.5%)、Postsilon波19例(79.2%)。18例(75%)出现右胸导联广义Epsilon波,主要为Postsilon波。结论广义Epsilon波可明显提高致心律失常性右室心肌病的诊断敏感性;广义Epsilon波亦常见于ARVC患者的右胸导联,且各亚型中以Postsilon波多见。Objective To compare the sensitivity and value of generalized Epsilon waves and Epsilon wave in the diagnosis of arrhythmogenic right ventricular cardiomyopathy.Methods The data of patients with arrhythmogenic right ventricular cardiomyopathy in the digital medical record system of Peking University People’s Hospital,CNKI(China National Knowledge Infrastructure)and Pubmed database from January 2009 to November 2020 were collected.The diagnostic sensitivity of Epsilon wave and Generalized Epsilon waves were compared,and the detection rate and lead distribution of Generalized Epsilon waves were analyzed.Results A total of 24 patients were collected,among them Epsilon waves were detected in 4 cases,while Generalized Epsilon waves were detected in 19 cases.The diagnostic sensitivity was 16.7%and 79.2%,Pearsonχ^(2)=18.783,p=0.000.Generalized Epsilon waves including 5 cases of Presilon wave(20.8%),3 cases of Topsilon wave(12.5%)and 19 cases of Postsilon wave(79.2%).Generalized Epsilon waves were found in right pectoral leads in 18 patients(75%),mainly Postsilon wave.Conclusions Generalized Epsilon waves could significantly improve the diagnostic sensitivity of patients with arrhythmogenic right ventricular cardiomyopathy.Generalized Epsilon waves mostly exists in the right pectoral leads,and Postsilon wave are the most common.
