改良可调式皮肤牵张闭合器与自制皮肤牵张带治疗皮肤软组织缺损对疼痛及瘢痕恢复的影响

摘　　要：目的探究改良可调式皮肤牵张闭合器与自制皮肤牵张带治疗皮肤软组织缺损对疼痛及瘢痕恢复的影响。方法回顾性选取2017年3月至2020年5月本院收治的皮肤软组织缺损患者76例。根据患者治疗方式不同分为研究组与对照组,各40、36例。对照组患者采取自制皮肤牵张带治疗,研究组患者采取改良可调式皮肤牵张闭合器治疗,观察两组患者疗效、不同时点VAS及瘢痕恢复情况差异。结果广义估计方程分析结果显示:提示研究组患者治疗后治疗有效率显著高于对照组(P<0.05);重复测量方差分析显示,两组患者VAS评分、血管分布、瘢痕厚度、瘢痕色泽、瘢痕柔软的时点、组间、VAS评分及瘢痕色泽的时点与组别的交互效应均差异有统计学意义(P<0.05)。治疗前,两组患者VAS评分比较差异无统计学意义(P>0.05);治疗3d、5d、7d后,研究组VAS评分均低于对照组,差异有统计学意义(P<0.05),且治疗1周后、2周后、3周后,研究组血管分布、瘢痕厚度、瘢痕色泽、瘢痕柔软分值均低于对照组,差异有统计学意义(P<0.05)。结论改良可调式皮肤牵张闭合器治疗皮肤软组织缺损效果明显,可有效降低患者疼痛,促进瘢痕恢复,值得临床应用。Objective To explore the effect of modified adjustable skin distraction closure and self-made skin distraction zone on pain and scar recovery in the treatment of skin soft tissue defect.Methods From March 2017 to May 2020,76 patients with skin and soft tissue defects were selected retrospectively.The patients were divided into study group and control group,40 and 36 cases each.Control group patients were treated with self-made skin distraction tape,study group patients were treated with modified adjustable skin distraction closure,observe the difference of curative effect,VAS at different time points and scar recovery between the two groups.Results The results of generalized estimation equation analysis showed that the effective rate of treatment in the study group was significantly higher than that in the control group(P<0.05),and the interaction effect between VAS score,vascular distribution,scar thickness,scar color,scar soft time point,group,VAS score and scar color was statistically significant(P<0.05).There was no significant difference in VAS score between the two groups before treatment(P>0.05).After 3 d、5d、7d of treatment,the VAS score of the study group was lower than that of the control group(P<0.05).After 1 week,2 weeks and 3 weeks,the blood vessel distribution,scar thickness,scar color and scar softness were lower than those of the control group(P<0.05).Conclusion The effect of modified adjustable skin distraction closure on skin and soft tissue defect is obvious,which can effectively reduce pain and promote scar recovery,which is worthy of clinical application.
