B超引导下右颈内静脉穿刺在头盆环牵引患者中的可行性研究

摘　　要：目的研究B超引导下右颈内静脉穿刺术在头盆环牵引患者中的安全性和有效性。方法2019年1~9月我院戴有头盆环牵引拟行脊柱矫形手术的患者75例,在B超引导下行右颈内静脉穿刺40例(A组),徒手穿刺35例(B组)。记录两组穿刺的成功率、穿刺时间、穿刺次数及出现并发症的例数,同时测量右颈内静脉内径的大小、颈内静脉与动脉的关系以及有无异常颈内静脉。结果A组患者全部穿刺成功,1例误穿动脉,穿刺时间(10.98±1.609)min,穿刺次数为(1.10±0.304)次;B组患者24例穿刺成功,余改为B超引导下穿刺成功,14例误穿动脉,穿刺时间(18.83±5.244)min,穿刺次数(2.57±1.037)次。两组穿刺成功率、穿刺次数及并发症情况比较,差异均有统计学意义(P<0.05)。75例患者平均内径为(0.9±0.5)cm,7例患者存在动静脉解剖异常。结论B超引导下右颈内静脉穿刺术在头盆环牵引患者中使用,穿刺成功率高,并发症少,安全有效。Objective To investigate the safety and efficacy of B-ultrasoundguided right internal jugular vein puncture in patients with head-basin-ring traction.Methods From January to September 2019,75 patients in our hospital were subjected to spinal orthopedic surgery by traction with pelvic ring.Of the patients,40 were subjected to right jugular vein puncture under B-ultrasound guidance(group A),and 35 were subjected to freehand puncture(group B).The success rate,puncture time,number of puncture needles and the number of complications in the two groups were recorded.Meanwhile,the size of the internal diameter of the right internal jugular vein,relationship between the internal jugular vein and artery,and the presence or absence of abnormal internal jugular vein was measured.Results All the patients in the group A were successfully punctured,including 1 patient who accidentally punctured the artery.The puncture time was(10.98±1.609)min,and the number of puncture needles was(1.10±0.304)times.In the group B,24 patients were successfully punctured and others were successfully punctured under B-ultrasound guidance.In the group B,14 patients were accidentally punctured.The puncture time was(18.83±5.244)min and number of puncture needles was(2.57±1.037)times.The difference of puncture success rate,number of puncture needles and complications between the two groups was statistically significant(P<0.05).The mean diameter of 75 patients was(0.9±0.5)cm,and 7 patients had arteriovenous anatomic abnormalities.Conclusion B-ultrasoundguided right jugular vein puncture is safe and effective in patients with head-basin-ring traction.
