腕踝针联合易罐治疗股骨粗隆间骨折高龄患者术后肿胀的临床疗效观察

摘　　要：目的观察腕踝针联合易罐治疗股骨粗隆间骨折高龄患者术后肿胀的临床疗效。方法选择2020年1—12月本院收治的80例股骨粗隆间骨折术后肿胀高龄患者作为研究对象,采用随机数表法分为对照组和治疗组两组,每组各40例。对照组采用传统术后消肿措施治疗,治疗组采用腕踝针联合易罐治疗,两组患者均治疗7天。比较两组患者的临床疗效、血流变学指标、肿胀程度、视觉模拟评分法(VAS)评分。结果治疗7天后,治疗组患者的临床疗效总有效率高于对照组,差异有统计学意义(P<0.05);治疗7天后,两组患者的凝血酶原时间均长于治疗前,D-二聚体水平均低于治疗前,差异有统计学意义(P<0.05);且治疗组患者的凝血酶原时间长于对照组,D-二聚体水平低于对照组,差异有统计学意义(P<0.05);术后第4、7天时,治疗组患者患肢的肿胀程度均低于对照组,差异有统计学意义(P<0.05);术后第4、7天时,治疗组患者患肢的VAS评分均低于对照组,差异有统计学意义(P<0.05)。结论腕踝针联合易罐治疗股骨粗隆间骨折高龄患者术后肿胀的疗效确切,可以改善血流变学指标,显著减轻患者的肿胀程度和疼痛感。Objective To observe the clinical effect of wrist and ankle acupuncture combined with cupping therapy in the treatment of postoperative swelling in elderly patients with intertrochanteric fracture.Methods A total of 80 elderly patients with swelling after surgery for intertrochanteric fracture those admitted to our hospital from January to December in 2020 were selected as the research subjects.The patients were randomly divided into two groups:control group and treatment group according to random number table method,with 40 cases in each group.The control group was treated with traditional postoperative detumescence measures,and the treatment group was treated with wrist and ankle acupuncture combined with cupping therapy.Both groups were treated for 7 days.Clinical efficacy,hemorrheology indexes,swelling degree,visual analogue scale(VAS)score were compared between 2 groups.Results After 7 days of treatment,the total effective rate of the treatment group was higher than that of the control group,the difference was statistically significant(P<0.05).After 7 days of treatment,the prothrombin time in both two groups were longer than those before treatment,and the D-dimer level was lower than before treatment,with statistical significance(P<0.05).The prothrombin time in treatment group was longer than that in control group,and the level of D-dimer in treatment group was lower than that in control group,the differences were statistically significant(P<0.05).On the 4th and 7th day after surgery,the swelling degree of the affected limb in the treatment group was lower than that in the control group,the difference was statistically significant(P<0.05).On the 4th and 7th day after surgery,the VAS scores of the affected limbs in the treatment group were lower than those in the control group,with statistical significance(P<0.05).Conclusions Wrist-ankle acupuncture combined with cupping therapy is effective in the treatment of postoperative swelling in elderly patients with intertrochanteric fracture,which could improve the
