补肾滋阴汤治疗卵巢储备功能低下不孕症的临床研究

摘　　要：目的观察卵巢储备功能低下不孕症应用补肾滋阴汤治疗的临床疗效。方法60例卵巢储备功能低下不孕症患者作为研究对象,依照治疗方式不同分为对照组和联合治疗组,每组30例。对照组采用常规激素治疗,联合治疗组在对照组基础上联合中药补肾滋阴汤治疗。比较两组治疗效果,治疗前后血清激素水平,治疗前后中医症候积分,月经恢复率及妊娠率。结果联合治疗组痊愈8例、显效12例、有效8例、无效2例,对照组痊愈4例、显效10例、有效6例、无效10例,联合治疗组的治疗效果优于对照组,差异有统计学意义(U=2.155,P<0.05)。治疗后,联合治疗组的促黄体生成素、雌二醇、孕酮、卵泡刺激素水平分别为(21.4±7.6)U/L、(155.7±33.6)pg/ml、(1.3±0.3)nmol/L、(24.5±5.1)U/L,均显著低于对照组的(31.5±7.2)U/L、(315.8±50.4)pg/ml、(6.0±1.2)nmol/L、(36.6±5.4)U/L,差异均有统计学意义(P<0.05)。治疗后,联合治疗组腰骶酸痛、小腹胀痛、性欲减退、头晕眼花积分分别为(0.7±0.3)、(0.7±0.1)、(0.9±0.2)、(0.6±0.3)分,均明显低于对照组的(1.2±0.4)、(1.1±0.5)、(1.2±0.3)、(1.1±0.4)分,差异有统计学意义(P<0.05)。联合治疗组的月经恢复率93.3%、妊娠率46.7%均明显高于对照组的66.7%、20.0%,差异均有统计学意义(χ^(2)=6.667、4.800,P<0.05)。结论补肾滋阴汤结合激素治疗卵巢储备功能低下不孕症可显著提升治疗效果,促进患者血清激素水平及中医症候积分降低,提高妊娠率及月经恢复率,效果较好。Objective To observe the clinical efficacy of Bushen Ziyin Decoction in treating infertility with diminished ovarian reserve.Methods A total of 60 cases of infertility with diminished ovarian reserve as study subjects were divided into control group and combined treatment group according to different treatment methods,with 30 cases in each group.The control group was treated with conventional hormone,and the combined treatment group was treated with Chinese medicine Bushen Ziyin Decoction.The therapeutic effect,serum hormone and Chinese syndrome score before and after treatment,menstrual recovery rate and pregnancy rate were compared between the two groups.Results In combined treatment group,there were 8 cure cases,12 excellent cases,8 effective cases and 2 ineffective cases;in control group,there were 4 cure cases,10 excellent cases,6 effective cases,and 10 ineffective cases.The total effective rate of combined group was better than that of the control group,and the difference was statistically significant(U=2.155,P<0.05).After treatment,the luteinizing hormone,estradiol,progesterone and follicle stimulating hormone of combined treatment group were(21.4±7.6)U/L,(155.7±33.6)pg/ml,(1.3±0.3)nmol/L and(24.5±5.1)U/L,which were significantly lower than(31.5±7.2)U/L,(315.8±50.4)pg/ml,(6.0±1.2)nmol/L and(36.6±5.4)U/L of the control group,and the difference was statistically significant(P<0.05).After treatment,the lumbosacral soreness,abdominal distention,hyposexuality,dizziness of combined treatment group were(0.7±0.3),(0.7±0.1),(0.9±0.2)and(0.6±0.3)points,which were obviously lower than(1.2±0.4),(1.1±0.5),(1.2±0.3)and(1.1±0.4)points of the control group,and the difference was statistically significant(P<0.05).The menstrual recovery rate 93.3%and pregnancy rate 46.7%of combined treatment group were obviously higher than 66.7%and 20.0%of the control group,and the difference was statistically significant(χ^(2)=6.667,4.800;P<0.05).Conclusion Combination of Bushen Ziyin Decoction and hormone shows good effe
