骨水泥型与生物型人工半髋置换术治疗高龄股骨颈骨折的疗效比较

摘  要:目的对高龄股骨颈骨折患者采用骨水泥型与生物型人工半髋置换术治疗的效果进行分析。方法82例高龄股骨颈骨折患者,采用计算机随机配号方法分为对照组和研究组,每组41例。研究组采用骨水泥型人工半髋置换术治疗,对照组患者采用生物型人工半髋置换术治疗。对比两组患者髋关节Harris评分、视觉模拟评分法(VAS)评分、手术指标以及术后并发症发生率。结果术后1个月、半年,研究组患者髋关节Harris评分分别为(62.03±2.84)、(92.03±3.00)分,高于对照组的(54.12±2.78)、(80.45±3.04)分,差异有统计学意义(P<0.05)。研究组患者术后2、24、48 h的VAS评分低于对照组,差异有统计学意义(P<0.05)。研究组患者手术时间(71.36±11.25)min、住院时间(17.05±3.69)d短于对照组的(93.03±10.85)min、(22.02±2.74)d,手术出血量(267.03±95.02)ml少于对照组的(379.85±80.74)ml,差异有统计学意义(P<0.05)。研究组患者并发症发生率为7.32%,低于对照组的24.39%,差异有统计学意义(P<0.05)。结论针对高龄股骨颈骨折患者采用骨水泥型人工半髋置换术治疗效果明显优于生物型人工半髋置换术,能够有效改善患者骨折症状,提高手术效率,缓解患者疼痛度,避免术后出现各种并发症。Objective To analyze the effect of cemented and biological hemiarthroplasty in the treatment of elderly femoral neck fractures.Methods A total of 82 elderly patients with femoral neck fractures were divided into control group and research group according to computer random method,with 41 cases in each group.The research group was treated with cemented hemiarthroplasty,and the control group was treated with biological hemiarthroplasty.The Harris score of hip joint,visual analogue scale(VAS)score,surgical index and occurrence of postoperative complications were compared between the two groups.Results At 1 month and half year after operation,the Harris score of hip joint of the research group were(62.03±2.84)and(92.03±3.00)points,which were higher than(54.12±2.78)and(80.45±3.04)points of the control group,and the difference was statistically significant(P<0.05).The VAS scores at 2,24 and 48 h after operation of the research group were lower than those of the control group,and the difference was statistically significant(P<0.05).The operation time(71.36±11.25)min and hospitalization time(17.05±3.69)d of the research group were shorter than(93.03±10.85)min and(22.02±2.74)d of the control group,and intraoperative blood loss(267.03±95.02)ml was less than(379.85±80.74)ml of the control group.All the difference was statistically significant(P<0.05).The incidence of complications of the research group was 7.32%,which was lower than 24.39%of the control group,and the difference was statistically significant(P<0.05).Conclusion Cemented hemiarthroplasty shows obvious effect than biological hemiarthroplasty in the treatment of elderly patients with femoral neck fractures,which can effectively improve the fracture symptoms,improve surgical efficiency,relieve pain of patients and avoid postoperative complications.
