复方苦参凝胶辅助治疗妊娠期细菌性阴道病的临床研究分析

摘　　要：目的探讨复方苦参凝胶辅助治疗妊娠期细菌性阴道病(BV)的临床效果。方法选取2016年10月至2018年1月首都医科大学附属复兴医院诊治的130例妊娠期BV患者作为研究对象。采用随机数字表法将其分为联合组(n=65)和对照组(n=65)。对照组给予甲硝唑栓和乳杆菌活菌胶囊的常规治疗,联合组在常规治疗基础上加用复方苦参凝胶,均治疗1周。对比两组治疗后阴道菌群Chao1指数和Shannon指数水平、临床疗效、不良反应、复发情况及不良妊娠结局情况。结果治疗后联合组阴道菌群Chao1指数和Shannon指数均高于对照组(P<0.05);联合组总有效率为92.31%,与对照组78.46%相比显著较高(P<0.05);治疗期间两组不良反应发生率相近(P>0.05);停药1个月,联合组复发率为6.15%,与对照组23.08%相比显著较低(P<0.05);联合组胎膜早破、早产、新生儿感染及低体重儿不良妊娠结局发生率与对照组相比显著较低(P<0.05)。结论复方苦参凝胶辅助治疗妊娠期BV效果显著,可显著改善阴道菌群丰富度,减少复发及不良妊娠结局情况。Objective To investigate the effect and role of compound matrine gel in the adjuvant treatment of bacterial vaginosis(BV)during pregnancy.Methods 130 patients with BV during pregnancy treated in Fuxing Hospital of Capital Medical University from October 2016 to January 2018 were selected as the research subjects,and they were divided into a combined group(n=65)and a control group(n=65)by random number table method.The control group was given conventional treatment with metronidazole suppositorium and live Lactobacillus capsule,while the combined group was given compound matrine gel on the basis of conventional treatment for 1 week.The levels of Chao1 index and Shannon index of vaginal flora,clinical efficacy,adverse reactions,recurrence and adverse pregnancy outcomes were compared between the two groups after treatment.Results After treatment,the Chao1 index and Shannon index of vaginal flora in combination group were higher than those in control group(P<0.05).The total effective rate of combined group was 92.31%,which was significantly higher than that of control group(78.46%,P<0.05).The incidence of ADR in the two groups was similar during treatment(P>0.05).After 1 month of drug withdrawal,the recurrence rate of combined group was 6.15%,which was significantly lower than that of control group(23.08%,P<0.05).Compared with the control group,the incidence of premature rupture of membranes,premature delivery,neonatal infection and adverse pregnancy outcomes of low birth weight infants in the combined group were significantly lower(P<0.05).Conclusions The adjuvant effect of compound matrine gel in the adjuvant treatment of BV during pregnancy is significant,which can significantly improve the richness of vaginal flora and reduce the recurrence and adverse pregnancy outcomes.
