摘 要:目的探究64排增强CT增强灌注成像对肝硬化、小肝癌的临床诊断价值。方法选取2016年2月至2018年4月我院放射科64排螺旋CT检查疑似小肝癌患者30例作为研究对象,同期体检健康者7例作为对照组。均行肝脏多期增强扫描及CT灌注扫描,观察小肝癌病灶,肝硬化、小肝癌灌注参数,分析小肝癌和背景肝关系。结果 16例行双动脉期增强扫描,共发现20个小肝癌病灶,综合分析双动脉期扫描小肝癌检出率90.0%。14例行单动脉期增强扫描,共发现16个小肝癌病灶,小肝癌检出率81.3%。肝硬化不同程度比较,BV、BF值随着肝硬化程度加重呈现减小,而HAF、PS、MTT、HAP值增大,肝硬化组HAF、HAP值明显高于对照组(P<0.05)。小肝癌组中,PS、HAF、HAP值随着小肝癌分化程度越差逐渐增大(P<0.05)。小肝癌恶性程度随着肝硬化程度加重而加重,同时分化程度逐渐变差。结论 64排增强CT增强灌注成像有助于评估、预测肝硬化程度及小肝癌分化程度,同时肝硬化程度与小肝癌分化程度相关。Objective To explore the clinical diagnostic value of 64-slice enhanced CT perfusion imaging in cirrhosis and small hepatocellular carcinoma.Methods Thirty patients suspected of small hepatocellular carcinoma were examined by 64-slice spiral CT from February 2016 to April 2018 in the radiology department of our hospital.Seven healthy persons in the same period served as a control group.All patients underwent a multi-phase contrast-enhanced scan and CT perfusion scan to observe the small hepatocellular ca rcinoma lesions,cirrhosis and small hepatocellular carcinoma perfusion parameters,and to analyze the relationship between small hepatocellular ca rcinoma and background liver Results Twenty small hepatocellular ca rcinoma lesions were found in 16 patients who underwent dual-a rterial phase contrast-enhanced sca n ning.Comprehensive analysis of the detection rate of small liver cancer in the double a rterial phase was 90.0%.A total of 16 small hepatoca rcinoma lesions were found in 14 patients with a single a rterial phase enhancement sca n,and the detection rate was 81.3%.Compa red with different degrees of cirrhosis,the values of BV and BF decreased with the severity of cirrhosis,while the values of HAF,PS,MTT,and HAP increased.The values of HAF and HAP in the cirrhosis group were significantly higher than those in the control group(P<0.05).In a small hepatocellular carcinoma group,the values of PS,HAF and HAP gradually increased with the differentiation of small hepatocellular carcinoma(P<0.05).The degree of malignancy of small hepatocarcinomawas aggravated with the increased severity of cirrhosis,and the degree of differentiation gradually decreased.Conclusion 64-slice enhanced CT perfusion imaging helps assess and predict the degree of cirrhosis and differentiation in small hepatocellular carcinoma.Simultaneously the degree of liver cirrhosis correlated with the degree of diffe rentiation of small hepatocellular ca rcinoma.
