摘　　要：目的分析不同TNM分期胃癌多层螺旋CT灌注参数与血清VEGF相关性。方法回顾性分析我院2017年6月至2019年7月收治的69例胃癌患者的临床资料,将MSCT检查和VEGF测定结果进行讨论和分析,比较胃癌患者不同TNM分期之间的灌注参数及血清VEGF值,对不同TNM分期胃癌组织灌注参数与血清VEGF含量的相关性进行分析。结果Ⅰ~Ⅱ期患者的BF、BV、MTT、PS值均低于Ⅲ~Ⅵ期患者,其中Ⅰ~Ⅱ期与Ⅲ~Ⅵ期的BF、PA值比较差异具有统计学意义(P<0.05);而MTT、PS值比较差异无统计学意义(P>0.05);Ⅲ~Ⅵ期胃癌患者的血清VEGF值显著高于Ⅰ~Ⅱ期胃癌患者(P<0.05);Ⅰ~Ⅱ期和Ⅲ~Ⅵ期胃癌患者灌注参数BF、PS值与血清VEGF含量之间均呈正相关,且相关性显著(P<0.05)。结论 MSCT灌注成像能反映胃癌的血流动力学变化,利用BF、PS能间接反映血清VEGF表达水平,对胃癌手术前评估具有重要的临床价值。Objective To analyze the correlation between perfusion parameters of gastric cancer in different TNM stages in multi-slice spiral CT and serum VEGF.Methods The clinical data of 69 patients with gastric cancer admitted to our hospital from June 2017 to J uly 2019 were retrospectively analyzed.The results of MSCT and VEGF were discussed and analyzed.The perfusion parameters and serum VEGF values of patients with gastric cancer in different TNM stages were compared.The correlation between perfusion parameters and serum VEGF levels of gastric cancer tissues in different TNM staging was analyzed.Resul;ts The BF,BV,MTT,and PS values of patients in stageⅠ-Ⅱwere lower than those of patients in stageⅢ-Ⅵ.The difference in BF and PA between stageⅠ-Ⅱand stageⅢ-Ⅵwas statistically significant(P<0.05).There was no significant difference in MTT and PS values(P>0.05).The serum VEGF value of patients with gastric cancer inⅢ-Ⅵstage was significantly higher than that of patients with gastric cancer inⅠ-Ⅱstage(P<0.05).There was a positive correlation between BF and PS values and serum VEGF levels in patients with gastric cancer inⅠ-ⅡandⅢ-Ⅳstage,and the correlation was significant(P<0.05).Conclusion MSCT perfusion imaging ca n reflect the hemodynamic changes of gastric cancer.BF and PS can reflect the expression level of serum VEGF indirectly,which has important clinical value for preope rative evaluation of gastric cancer.
