摘　　要：目的分析我院2019年1~8月门急诊西药房退药原因,探讨如何减少退药,保障患者用药安全。方法统计我院2019年1~8月门急诊西药房的退药记录,使用帕雷托图分析引起退药的主要因素、次要因素和一般因素。结果引起退药的主要因素有患者要求退药、医嘱停止、录入错误、数量差错、重复用药和不良反应;次要因素有转住院、麻醉精神药品数量超限和药房缺药;一般因素有选错厂家、已备药、溶剂选择不对、对价格或报销不满意、非正常班时间不能取麻醉精一、门诊不得输液和药品质量问题。结论为减少门急诊西药房的退药,应加强医生、患者和药师的沟通,发挥药师的作用,加强医院信息化建设,完善退药制度。OBJECTIVE To analyze the reasons for drug repercussion in outpatient and emergency western pharmacy of our hospital from January to August 2019,explored how to reduce the drug repercussion,ensured the secure medication of patient.METHODS To summarize the drug repercussion records of outpatient and emergency pharmacy in our hospital from January to August 2019,used the pareto diagram to analyze the main factors,secondary factors and general factors causing drug repercussion.RESULTS The main factors causing drug repercussion included patient′s request for drug repercussion,doctor′s order to stop,input error,quantity error,repeated the drugs and ADR.The secondary factors included hospitalization,exceeded the limit of the narcotic drugs and psychotropic drugs and drug shortage in the pharmacy.The general factors included improper manufacturer selection,prepared the drugs,improper solvent selection,not satisfied with the price or reimbursement,can′t take the narcotic drugs and psychotropic drugs in abnormal shift time,no infusion in outpatient department and the quality problems of drugs.CONCLUSION To reduce the drug repercussion of outpatient and emergency pharmacy,strengthen the communication between doctors,patients and pharmacists,play the role of the pharmacist,strengthen the construction of the hospital informatization,improve the system of drug repercussion.
