经皮肾镜钬激光碎石术治疗复杂性肾结石效果及对肾功能的影响  

Percutaneous Nephrolithotomy with Holmium Laser Lithotripsy in the Treatment of Complicated Renal Calculi and Its Influence on Renal Function

摘　　要：目的:探讨经皮肾镜钬激光碎石术治疗复杂性肾结石疗效及对肾功能的影响。方法:选取2019年1月-2020年6月笔者所在医院收治的复杂性肾结石患者98例,根据随机数字表分为对照组(49例)与研究组(49例),对照组患者治疗予肾切开取石术,研究组患者治疗予经皮肾镜钬激光碎石术,比较两组患者治疗前后肾功能[血尿素氮(BUN)、血肌酐(Scr)、脂质运载蛋白(NGAL)、半胱氨酸蛋白酶抑制剂(CysC)]、治疗效果与术后并发症。结果:治疗前两组患者肾功能比较差异无统计学意义(P>0.05),治疗后1周两组患者各肾功能指标水平上升,治疗后4周各肾功能指标水平逐渐下降,并且治疗后1、4周研究组患者BUN、Scr、NGAL、CysC均明显低于对照组(P<0.05);较对照组,研究组治疗总有效率明显更高(83.67% vs 95.92%,P<0.05);较对照组,研究组术后并发症率明显更低(24.49% vs 10.20%,P<0.05)。结论:复杂性肾结石患者采取经皮肾镜钬激光碎石术治疗对其肾功能影响较小,有利于术后肾功能恢复,治疗效果良好且并发症少,安全性较高。Objective:To investigate the effect of percutaneous nephrolithotomy with holmium laser lithotripsy in the treatment of complicated kidney stones and its influence on renal function.Method:A total of 98 patients with complicated kidney stones admitted to our hospital from January 2019 to June 2020 were selected and divided into a control group (49 cases) and a study group (49 cases) according to a random number table.The control group was treated with kidney.The patients in the study group were treated with percutaneous nephroscope holmium laser lithotripsy.The renal function of the two groups was compared before and after treatment[blood urea nitrogen (BUN),blood creatinine (Scr),lipocalin (NGAL),cysteine protease inhibitor (CysC)],therapeutic effect and postoperative complications.Result:The renal function of the two groups of patients compared before treatment has no significant difference (P>0.05).The levels of renal function indexes of the two groups of patients increased at 1 week after treatment,and the levels of renal function indexes gradually decreased at 4 weeks after treatment,and 1 week and 4 weeks after treatment BUN,Scr,NGAL,and CysC in the study group were significantly lower than those in the control group (P<0.05).Compared with the control group,the total effective rate of treatment in the study group was significantly higher (83.67% vs 95.92%,P<0.05),the postoperative complication rate was significantly lower (24.49% vs 10.20%,P<0.05).Conclusion:Percutaneous nephrolithotomy with holmium laser lithotripsy for patients with complex kidney stones has little effect on renal function,which is beneficial to the recovery of renal function after surgery.The treatment effect is good,there are few complications,and the safety is high.
