Gibson后外侧入路关节置换与空心螺钉内固定治疗老年移位性股骨颈骨折的临床效果

摘　　要：目的:探讨吉布森(Gibson)后外侧入路关节置换与空心螺钉内固定治疗老年移位性股骨颈骨折的临床效果。方法:选取2017年3月-2020年3月本院收治的老年移位性股骨颈骨折患者84例。采用随机数字表法分为对照组和观察组,各42例。对照组行空心螺钉内固定治疗,观察组行Gibson后外侧入路关节置换术。比较两组临床指标、髋关节功能及并发症情况。结果:观察组手术时间、住院时间长于对照组,出血量多于对照组,术后24 h疼痛评分高于对照组,术后不同时点髋关节功能评分均高于对照组,并发症发生率(4.76%)低于对照组(21.43%),差异均有统计学意义(P<0.05)。结论:与空心螺钉内固定相比,老年移位性股骨颈骨折患者采用Gibson后外侧入路关节置换术治疗创伤更大,但术后髋关节功能恢复更好,并发症更少,临床可根据患者实际情况进行选择。Objective:To investigate the clinical efficacy of Gibson posterolateral approach joint replacement and hollow screw internal fixation in the treatment of displaced femoral neck fractures in the elderly.Method:A total of 84 elderly patients with displaced femoral neck fractures admitted to our hospital from March 2017 to March 2020 were selected and divided into the control group and the observation group by random number table,with 42 cases in each group.The control group underwent hollow screw internal fixation,and the observation group underwent Gibson posterolateral approach joint replacement.The clinical indicators,hip function and complications of the two groups were compared.Result:The operation time and hospital stay of the observation group were longer than those of the control group,and the amount of bleeding was more than that of the control group,the pain score at 24 h after operation was higher than that of the control group,the hip function scores at different points after the operation were higher than those of the control group,the incidence of complications (4.76%) was lower than that of the control group (21.43%),the differences were statistically significant (P<0.05).Conclusion:Compared with the hollow screw internal fixation,elderly patients with displaced femoral neck fractures treated with Gibson posterolateral approach joint replacement are more traumatic,but the postoperative hip joint function recovery is better,and there are fewer complications.Clinical selection can be made according to the actual situation of patients.
