膀胱穿刺造瘘联合输尿管镜尿道会师术治疗后尿道断裂的临床观察

摘　　要：目的:观察膀胱穿刺造瘘联合输尿管镜尿道会师术治疗后尿道断裂情况。方法:选取笔者所在医院2015年4月-2020年2月收治的后尿道断裂男性患者64例,按随机数表法分为观察组和对照组,每组32例。观察组给予膀胱穿刺造瘘联合输尿管镜尿道会师术治疗,对照组给予尿道会师术治疗,比较两组患者手术中出血情况、手术时间、最大尿流率及并发症等情况。结果:观察组手术时间短于对照组,手术出血量少于对照组(P<0.05),两组最大尿流率及残余尿量比较差异无统计学意义(P>0.05);观察组排尿通畅率(93.75%)明显高于对照组(56.25%),差异有统计学意义(P<0.05);观察组术后感染2例,勃起功能障碍0例,尿道狭窄2例,并发症发生率为12.50%,对照组术后感染6例,勃起功能障碍2例,尿道狭窄4例,并发症发生率为37.50%,对照组并发症率明显高于观察组(P<0.05)。结论:膀胱穿刺造瘘联合输尿管镜尿道会师术治疗后尿道断裂用时短,出血量少,治疗效果明显。Objective:To observe the healing of vesicocentesis ostomy combined with ureteroscopy urethrologist of suprapubic local infiltration.Method:A total of 64 male patients with posterior urethral rupture treated in our hospital from April 2015 to February 2020 were selected and divided into the observation group and the control group,with 32 cases in each group,according to the random number table method.In the observation group,patients were given treatment by vesicocentesis ostomy combined with ureteroscopic urethroperineostomy.In the control group,patients were given urethroperineostomy.And bleeding during surgery,operation time,maximum urine output rate,and complications were compared between the two groups.Result:The operative time of the observation group was shorter than that of the control group,the surgical blood loss of the observation group was less than that of the control group (P<0.05);the maximum urine output and the residual urine volume were not significantly different between the two groups (P>0.05).The voiding patency rate of the observation group (93.75%) was significantly higher than 56.25% of the control group,the difference was statistically significant (P<0.05).In the observation group,there were 2 cases of postoperative infection,0 case of erectile dysfunction and 2 cases of urethral stricture,and the complication rate was 12.50%.In the control group,there were 6 cases of postoperative infection,2 cases of erectile dysfunction and 4 cases of urethral stricture,and the complication rate was 37.50%.The complication rate in the control group was significantly higher than that in the observation group (P<0.05).Conclusion:Vesicocentesis ostomy combined with ureteroscopic urethroperineostomy in the treatment of posterior urethral rupture takes a short time,less bleeding,and the treatment effect is obvious.
