慢性心力衰竭患者血清hs-CRP表达水平与心功能的相关性

摘　　要：目的:探讨慢性心力衰竭患者血清超敏C反应蛋白(hs-CRP)表达水平与心功能的相关性。方法:选择2018年5月-2020年5月本院收治的75例慢性心力衰竭患者为研究对象,将其纳入CHF组,选择本院同期收治的心功能正常患者75例,将其纳入对照组,CHF组参照美国纽约心脏病协会(NYHA)分级分为NYHA Ⅰ+Ⅱ级组、NYHA Ⅲ级组、NYHA Ⅳ级组。比较各组间心功能指标[左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)、6 min步行试验(6MWT)]及hs-CRP水平,并分析hs-CRP表达水平与心功能的相关性。结果:CHF组LVEDD、hs-CRP水平均高于对照组,LVEF、6MWT低于对照组,差异有统计学意义(P<0.05);NYHA Ⅰ+Ⅱ级组LVEF、6MWT水平高于NYHA Ⅲ级组、NYHA Ⅳ级组,LVEDD、hs-CRP水平低于NYHA Ⅲ级组、NYHA Ⅳ级组,差异有统计学意义(P<0.05)。经Pearson相关性分析结果显示,LVEF、6MWT水平与血清hs-CRP水平均呈负相关(r<0,P<0.05);LVEDD水平与血清hs-CRP水平均呈正相关(r>0,P<0.05)。结论:血清hs-CRP与心功能具有一定相关性,可作为慢性心力衰竭患者诊断与判断预后的相关指标,值得临床推广。Objective:To investigate the correlation between the expression level of serum high sensitive C reactive protein (hs-CRP) and cardiac function in patients with chronic heart failure.Method:A total of 75 patients with chronic heart failure admitted to our hospital from May 2018 to May 2020 were selected as the research subjects and included in the CHF group,75 patients with normal cardiac function admitted to our hospital during the same period were selected and included in the control group,the CHF group was divided into the NYHA Ⅰ and Ⅱ group,NYHA Ⅲ group and NYHA Ⅳ group according to New York heart disease association (NYHA) classification.The cardiac function indexes[left ventricular ejection fraction (LVEF),left ventricular end diastolic diameter (LVEDD),6 min walk test (6MWT)]and hs-CRP levels were compared among the groups,the correlation between the expression level of hs-CRP and cardiac function was analyzed.Result:The levels of LVEDD and hs-CRP in the CHF group were higher than those in the control group,LVEF and 6MWT in the CHF group were lower than those in the control group,the differences were statistically significant (P<0.05);the levels of LVEF and 6MWT in the NYHA Ⅰ and Ⅱ group were higher than those in the NYHA Ⅲ group and the NYHA Ⅳ group,LVEDD and hs-CRP levels were lower than those in the NYHA Ⅲ group and the NYHA Ⅳ group,the differences were statistically significant (P<0.05).Pearson correlation analysis showed that LVEF and 6MWT were negatively correlated with serum hs-CRP level (r<0,P<0.05);LVEDD level was positively correlated with serum hs-CRP level (r>0,P<0.05).Conclusion:Serum hs-CRP has a certain correlation with cardiac function,which can be used as a related index for the diagnosis and prognosis of patients with chronic heart failure,which is worthy of clinical promotion.
