计划性饮食能量管理对阴道分娩产妇产程进展的影响

摘　　要：目的探讨以新产程标准为基础的计划性饮食能量管理对阴道分娩产妇产程进展的作用效果。方法将210例足月无痛分娩初产妇随机分为干预组和对照组,各105例,对照组采取自由饮食,干预组实施计划性饮食能量管理。比较2组分娩时间、血液生化指标及新生儿结局情况。结果干预组第一产程时间、总产程时间均短于对照组(t=3.703,P<0.001;t=4.313,P<0.001),2组第二产程时长比较,差异无统计学意义(P>0.05);分娩后,干预组产妇血糖、血钠,较对照组呈现更高的水平(t=3.880,P=0.023;t=2.759,P=0.038);2组分娩的新生儿脐静脉血气分析中的pH、乳酸、碱剩余值及低血糖发生率比较,差异均无统计学意义(P>0.05)。结论计划性饮食能量管理模式有利于促进产妇分娩第一产程进展,保障产妇能量需求,提高产妇体能,且不影响新生儿结局。Objective To explore the effect of planned dietary energy management labor progress of women with vaginal delivery.Methods Totally 210 primiparas were randomly divided into an intervention group(n=105)and a control group(n=105).The control group was free to eat,while the intervention group was given planned diet energy management.The changes of uterine electromyography,delivery time and neonatal outcomes were compared between the two groups.Results The duration of the first-stage labor and total labor of the intervention group were significantly shorter than those of the control group(t=3.703,P<0.001;t=4.313,P<0.001),but there was no significant difference in that of the second-stage labor between the two groups(P>0.05).After delivery,the maternal blood sugar and blood sodium levels of the intervention group were significantly higher than those of the control group(t=3.880,P=0.023;t=2.759,P=0.038).There was no significant difference in the average residual values of pH,lactic acid,alkali and the incidence of hypoglycemia between the two groups according to the umbilical vein blood gas analysis(P>0.05).Conclusion Planned dietary energy management model can help to promote the progress of the first-stage labor,ensuring their energy needs,and benefit the neonatal outcomes.
