摘 要:目的探讨IgG4相关性疾病致冠状动脉病变的多层螺旋CT影像诊断价值。方法收集本中心确诊为IgG4相关性疾病致冠状动脉病变患者5例,均符合IgG4相关性疾病致冠状动脉病变的诊断标准,分析患者的影像资料。结果5例患者CT均表现冠状动脉管壁增厚,部分呈多发性,部分冠状动脉瘤样扩张,类似“槲寄生”。其中累及左前降支5例、对角支1例、左回旋支2例、右冠状动脉2例。血清IgG4水平均升高,为21.2~39.2 g/L。结论多层螺旋CT结合三维重建技术能很好的显示受累冠状动脉的影像学特点,同时结合临床病史及实验室检查依据,能够较准确的对IgG4相关性疾病致冠状动脉病变做出诊断,为临床进一步诊断及治疗提供很大帮助。Objective To study the value of multi-detector computed tomography(MDCT)imaging in diagnosis of coronary artery lesions caused by IgG4-related diseases.Methods Five patients with coronary artery lesions caused by IgG4-related diseases from the Third Medical Center were collected,the imaging data of patients were analyzed.Results MDCT showed the thicking coronary artery wall in the 5 patients with multiple coronary artery lesions or ectasia.Conclusion MDCT imaging combined with 3D reconstruction can effectively display the imaging characteristics of the involved coronary arteries and accurately diagnose coronary artery lesions caused by IgG4-related diseases according to their clinical history and laboratory testing parameters,thus greatly helping their further clinical diagnosis and treatment.
